This position is responsible for the coordination of Population Health projects focused on a subset of members to meet quality and performance measure goals. Responsible for the development and implementation of data collection methodology to meet or exceed minimum performance standards established by the regulator, reporting and analysis of member outreach and social drivers of health program.  This position serves as a department and health plan resource for social drivers of health and member outreach while assisting with all programmatic needs of the Population Health model.

Primary Responsibilities:

  • Conduct member outreach to connect members to Population Health and quality improvement programs 
  • Assist with data collection for QI Pop Health Lead
  • Manage social drivers of health projects and conduct member screenings as assigned
  • Conduct member events and outreach for Population Health strategies
  • Develop materials for Population Health strategies 
  • Ensure that project schedules include all tasks and activities necessary to achieve project objectives
  • Ability to collaborate with and guide work of multidisciplinary teams
  • Assist with data collection for QI Pop Health Lead
  • Track project performance, specifically to analyze the successful completion

Years of post-high school education can be substituted/is equivalent to years of experience

Required Qualifications:

  • 3 years of relevant work experience in project management, member experience, health care, and/or QI
  • Intermediate level of proficiency with MS Word, Excel, PowerPoint and SharePoint 
  • Basic knowledge of social drivers of health and screening for health related social needs 
  • Proven intermediate problem solving and analytical skills
  • Proven excellent time management, organizational, and prioritization skills and ability to balance multiple priorities
  • Proven advanced interpersonal skills to effectively communicate both written and verbal with members, providers and health plan staff
  • Proven ability to maintain records of outreach and document clearly in a variety of programs
  • Proven ability to influence and work with a wide range of internal stakeholders/resources to move projects and programs forward and overcome any challenges
  • Located in the state of Wisconsin

Preferred Qualifications:

  • Project Management Professional (PMP) or similar Certification
  • Health care experience
  • Experience with HEDIS and NCQA
  • Experience working with Medicaid populations
  • Bilingual (English and Spanish)
