Are you a medical clinic coordinator looking to work for a company who appreciates you? The Positions in this function drive clinical relationships and engagement with physician practices, members, and pharmacies while partnering internally (with areas such as Network contract ACO mgrs, Health Care Economics and Analytics, Medical Directors, Reporting, Health Plan market leaders) with a goal of improving health, wellbeing, quality, and practice performance while reducing medical costs.
If you are located in the state of Arizona, you will have the flexibility to work remotely* as you take on some tough challenges.
Primary Responsibilities:
- Applies knowledge/skills to activities that often vary from day to day
- Demonstrates a moderate level of knowledge and skills in own function
- Requires little assistance with standard and non-standard requests
- Solves routine problems on own
- Works with supervisor to solve more complex nuisances
- Prioritizes and organizes own work to meet agreed upon deadlines
- Works with others as part of a team
- Full range of clinical practice performance which may include but is not limited to improvement on HCC/Risk, HEDIS and STARs gap closure, coding accuracy, facilitating effective education, and reporting, effective super utilizer engagement (e.g., members with complex and/or chronic conditions)
- Proactively identifying performance improvement
- Opportunities using data analytics,exel, technology, workflow changes and clinical support
- Develop comprehensive, provider-specific plans to increase their physician practice performance, reduce readmissions and improve their outcomes

Required Qualification:
- High school education or equivalent

- Bachelor's in Business or some college
- HCC Knowledge and Risk Adjustment
