This position entails serving as a clinical pharmacist while also performing staff pharmacist duties as necessary, depending on clinical requirements.
Duties and Responsibilities
• Provide and ensure excellent customer service to all pharmacy customers.
• Demonstrate a good working knowledge of the pharmacy management system.
• Demonstrate a good working knowledge of the practice management system and EMR and is responsible for utilizing this as part of the fill process for the purpose of reconciliation and adherence monitoring.
• Supervise the pharmacy technicians in the absence of the Pharmacist in Charge and with regards to the fill process and maintaining quality assurance throughout the process including accuracy of fills at all times: (patient, provider, NDC, dosing, refills, dates, signatures, directions, drug-drug interactions, drug-allergies interactions and DAW coding).
• Ensure the accuracy and safety of the medications dispensed and accuracy of the claim submissions.
• Ensure maintenance of patient records to include up-to-date allergies and medication reconciliation.
• Proper maintenance, storage and transportation of medications.
• Review of patient adherence and communicating deficiencies to the provider and making referrals to the nurse educator for adherence counseling or medication education.
• Communicate any concerns, discrepancies between the EMR medication list and the pharmacy system as well as updating the EMR with new prescriptions from third parties as needed. The care team should be advised via email of new prescriptions even though they are being added to the system.
• Ensure that all patients filling prescriptions have a follow-up appointment in the system. Notifications are to be sent at any point a patient does not have an appointment and a note placed on the bag to address directly with the patient when they come in to pick up.
• Counsel patients with new medications, adherence issues, high-risk medication, or upon request.
• Responsible for logging errors, monitoring and reviewing on a monthly basis.
• Working knowledge of insurance claims payments and claims resolution.
• Proficient in multiple disease states: HIV, Hepatitis C, CHF, Diabetes, COPD, Asthma, and Heart Disease.
• The pharmacist should advise providers when necessary on adjustments in medication selection, dosages, interactions, and side effects of medications.
• Coordination and supervision of deliveries and mailing of prescriptions.
• Inventory control and management. Ensure that all necessary medications are available; expired drugs are removed from stock; inventory numbers and accurate and up to date with correct NDC numbers.
• Answer pharmacy-related telephone calls and check voice messages consistently. All calls should be returned within the same day.
• Assist in the annual inventory process.
Minimum Qualifications
• Pharm D or Registered Pharmacist.
• 2-3 years’ experience in a similar role.
• Personal and professional passion to help improve healthcare delivery.

• Ability to build and maintain relationships with a diverse population.
• Team and customer service oriented. Working conditions This job operates in a professional pharmacy environment. This role routinely uses general office equipment.
Physical requirements

Job Type: Part-time
Pay: $55.00 - $62.00 per hour
Expected hours: 24 – 32 per week
