The UConn Health Women's Center for Preservation of Motion and Performance is seeking a highly motivated Nurse Navigator to join our multidisciplinary care team. The Nurse Navigator will serve a crucial role in patient care coordination, acting as the primary point of contact for patients and families. This position requires a high level of patient interaction, including triaging patient calls to schedule appointments with the appropriate team members, providing patient education, and conducting outreach initiatives to identify potential new patients.

SCHEDULE: This is a full-time salaried position, 40 hours per week. Monday through Friday 8:00 a.m. to 5:00 p.m., 60-minute unpaid meal break
EXAMPLES OF DUTIES:
  • Triage patient calls and coordinate the scheduling of appointments with the appropriate member of the multidisciplinary care team.
  • Provides patient education and maintains appropriate communication with medical, clinical, support and research staff, as well as the patients' family and caregivers

  • May introduce the concept of clinical investigator initiated studies and facilitates connection with clinical research personnel

  • Maintains the program's databases, quality standards and data management of key quality indicators

  • Participates in outcomes evaluation as guided by The Joint Commission (TJC) accreditation standard

  • Participates in program development, including but not limited to, staff education, community education, and marketing; assists in the planning, development and implementation of the program initiatives; serves as mentor to staff; participates in meeting and consults with health care institutions and community agencies to meet the needs of the patient; maintains clinical expertise and professionalism

  • Deliver comprehensive patient education about treatment plans and healthcare options, ensuring patients have the necessary information to make informed decisions.
  • Coordinates outreach initiatives to identify potential new patients and broaden the reach of the center.
  • Identify and distribute appropriate screening metrics for patient evaluation and monitoring.
  • Administer patient-reported outcome pathways, collecting and analyzing data to improve patient care outcomes.
  • Strive to continually improve patient care by identifying and reducing barriers to care.
  • Collaborate closely with physicians and interdisciplinary team members to triage, coordinate, and manage patient care consistently.
  • Serve as the primary point of contact for patients and families, providing a link to research and offering indirect patient care as needed.
MINIMUM QUALIFICATIONS REQUIRED:
KNOWLEDGE, SKILLS AND ABILITIES:
  • Considerable knowledge of the principles and practices of nursing.
  • Knowledge of case management principles and practices.
  • Knowledge of the Core measures for the specific discipline.
  • Knowledge of community resources; strong background in cultural competency and understanding of the cultural beliefs and traditions of various communities.
  • Computer proficiency; effective communication skills, both written and verbal; teaching skills; good organization skills with the ability to manage competing priorities in a timely manner.
  • Ability to work collaboratively with all members of the treatment team as well as community and payer representatives.
  • Ability to collect and analyze data; strong patient service/satisfaction focus; passion to care for people and commitment to healthcare.
EXPERIENCE AND TRAINING:
GENERAL EXPERIENCE:
  • Must possess and maintain a State of CT Registered Nurse (RN) license during employment.
  • Registered Nurse (RN) and three (3) years recent acute care experience in Orthopedics
  • Excellent communication and organizational skills, with an ability to manage multiple tasks simultaneously in a fast-paced environment.
  • Proficiency with electronic health records systems and digital scheduling platforms.
SUBSTITUTION:
  • Master's degree in Nursing may be substituted for one year of the required general experience.
PREFERRED EXPERIENCE:
  • Master's Degree in Nursing.
FULL TIME MINIMUM EQUIVALENT SALARY: $80,558.00
