About you:

The Clinical Operations Coordinator will support operations for the Care Management team as follows:

  • Responsible for data tracking and performance reporting
  • Facilitates flow of referrals from across the CareMax network
  • Serves as the point of contact between patients, physicians, patient care facilities, and clinical staff
  • Develops workflows and job aids for operational initiatives

Qualifications and Skills

  • Minimum 2 years of customer service experience or 1 year of project management, preferably in the healthcare industry
  • Experience working with referral management in physician’s office or for health plans preferred
  • Advanced-level skills in Microsoft 365 including Excel, PowerPoint, Visio, and Word
  • Ability to interpret healthcare data
  • Ability to multitask and work with multiple software applications
  • Strong communication and interpersonal skills
  • Electronic medical record experience preferred
  • Knowledge of healthcare concepts & programs, including Accountable Care Organizations operations, preferred

Essential Duties and Responsibilities

  • Data tracking/performance reporting
    • Updates performance tracker to report on trends
    • Updates weekly/monthly performance slide decks
    • Regular distribution of performance scorecards
    • Ad hoc reporting
  • Referral management
    • Verification of patient eligibility for Care Management programs
    • Daily creation of cases in Care Management documentation system (Innovaccer)
    • Daily creation of ADT notifications
  • Communication
    • Monitors Care Management phone line and email inbox, redirects and follows up as appropriate
    • Distributes patient mailings/correspondence to both patients and PCPs
  • Administrative responsibilities 
    • Maintains administrative documents, such as staff directory, organization chart, etc.
    • Provides information from electronic medical records of partners to care management team
    • Documents within care management documentation system
    • Develops workflows and job aids
    • Supports pilot initiatives
    • Performs other duties as assigned
    Regularly updates project plans for assigned tasks
