The Clinical Nurse Coordinator is accountable directly to the Director of Nursing. This position is responsible for supervision of a team of medical assistants as assigned at our Conway Health and Resource Center. This is a full-time position.
 
Salary- This position starts at $77.5k and up based on experience. Indeed provides their own estimated salary calculator and is not affiliated with COH's range.

Highlighted Duties and Responsibilities:
  • Serves as a registered nurse performing patient care and nursing administrative duties for patients in accordance with established policies, procedures, protocols, and guidelines.
  • Prepares and administers prescribed medications by oral, sublingual, intramuscular, intradermal, subcutaneous, topical, dermal, and inhalation routes.
  • Maintains familiarity with Infection Control OSHA Policies and helps monitor practices of staff.
  • Works as an integral part of the chronic care management team to assure that the patient receives appropriate and timely care, follow-up, and education.
  • Provides nursing care to patients including, but not limited to, EKGs, wound dressing, assisting providers with minor procedures, and administering nebulizer treatment to asthmatic patients as ordered by providers.
  • Meets with supervisees regularly and provides ongoing feedback and support, including opportunities for professional development.
  • Approves supervisee's time sheets per the established payroll deadlines.
  • Reviews the performance of team members in a timely manner, including completing 90-day and annual performance evaluations per their due dates and documenting any issues or changes.
  • Partners with the Talent Management team regarding personnel matters including but not limited to performance management, terminations, leaves of absence, recruiting, training, and onboarding.

Minimum Qualifications:
  • RN license required.
  • A current, unencumbered DC license with current CPR certification is required.
  • Knowledge of primary care and health maintenance is required.
  • A minimum of 1-2 years experience in an ambulatory care setting is required.
  • Experience with eClinical Works or other EHR preferred.
  • Commitment to work with underserved populations is required.
  • All Health Staff must receive the required vaccinations. This includes annual flu and covid vaccines.  Medical or religious exemptions may be requested.

    Community of Hope is a mission-driven, innovative, and rapidly growing nonprofit. For nearly 40 years, we have provided healthcare, housing, and supportive services for low-income, underserved, and homeless people in Washington DC. As a Federally Qualified Health Center, we provide medical, dental, behavioral health, and care coordination services for the whole family at three locations in DC. Community of Hope also has a strong emphasis on maternal and child health, with a midwifery practice and the only free-standing birth center in DC. In 2020, Community of Hope provided about 28,400 medical visits, 7,000 dental visits, and 10,000 behavioral health visits for about 11,000 patients. Community of Hope is also providing community walk-in COVID testing and COVID vaccines.
