Clinical Supervisor Home Health and Hospice
Salary: $75k to $90k based on experience
Duties:
-Receive case referrals. Review available patient information related to case, including home visits, to determine hospice/Home Health needs. Assigns appropriate hospice/Home Health personnel to case as needed. Conferences with the Director regarding any questions about an individual’s eligibility for services.
-Review and evaluates each case through a variety of means such as home visits, conferences, record review and the services provided by clinicians; discusses and verifies impressions, instructs and guides clinicians to promote more effective performance and delivery of quality hospice/Home Health services; and is available at all times during operative hours to assist clinicians as appropriate.
-Assist clinicians in establishing immediate and long-term therapeutic goals, in setting priorities, and in developing plan of care.
-Attend case conference meetings with hospice/Home Health personnel to facilitate coordination of care and discussion of interdisciplinary group involvement.
-Assist in the screening and interviewing process of new employees and makes recommendations for employment of individuals. Assist in the orientation of new employees.
-Assist Patient Care Coordinator/Clinical Director and other supervisory hospice/Home Health personnel in the planning, implementation and evaluation of in-service and continuing education programs. Assists in the formulating, revising, implementing and evaluating organization policies, procedures, goals and objectives, both short and long range.
-Assess health needs of community and assists in the development of plan(s)/programs(s) to meet needs. Participates in public relation and community activities that promote the organization’s role as an effective member of the hospice/Home Health delivery system. Promotes customer service orientation to all hospice/Home Health personnel.
-Participate actively in quality management program.
-Perform other duties and activities as delegated by the Patient Care Coordinator/Clinical Director.
Skills/Qualifications:
-Degree in Nursing from an approved school of nursing as determined by the Board of Nursing, required.
-Licensed to practice registered nursing in the state by the Board of Nursing.
-Previous experience in Home Health care setting
-Understands principles of Home Health
-Strong working knowledge of Home Health regulatory requirements
-Strong computer skills
-Strong leadership skills
Certificates, Licenses, Registrations:
- Current RN licensure in the state of OK, Home Health Administrator license preferred, CPR certification
