The Dozoretz Hospice House of Hampton roads is looking to hire a Full-Time Clinical Coordinator.

I. QUALIFICATIONS

  • Graduate from an accredited school of nursing, Bachelor’s Degree preferred.
  • Currently licensed as a registered nurse to practice in Virginia.
  • Minimum 2 years of experience in hospice care, combined with one year of supervisory experience.
  • Active patient contact within past three years, preferred.
  • Ability to work independently, make accurate, and at times, quick judgments.
  • Ability to collaborate with several parties to further hospice programs, including creating/amending policies.
  • Ability to supervise hospice clinical staff.
  • Ability to respond appropriately to crisis outside of a hospital setting.
  • Acceptance of and adaptability to different social, racial, cultural, and religious modes.
  • Demonstrated ability in or application of organizational/communication skills.
  • Ability to deal effectively with high levels of stress.
  • Prior experience using electronic health records, preparing/entering clinical notes.
  • Strong computer skills, including Microsoft and Google applications.
  • Ability to multi-task and meet deadlines.

II. RESPONSIBILITIES

  • Ensures compliance with federal, state, and local regulations, accreditation standards and hospice policies in all patient care aspects of the hospice program.
  • Participates in developing standards which ensure safe and therapeutically effective service to patients and families, including education on pain management. Has joint responsibility with the Administrator for seeing that standards are met.
  • Consults with the Administrator to determine a staffing pattern and assignments which will accomplish stated objectives and promote maximum utilization of personnel.
  • Responsible for recruiting, hiring, orientation of new hires, evaluating and terminating service personnel. Establishes criteria and procedures for selection, promotion and termination of employment of service personnel.
  • Participates in developing clear, achievable and specific objectives for hospice clinical staff and programs, ensuring competency per regulations.
  • Responsible for seeing that objectives are implemented, assuring that nursing services delivered are provided according to established hospice program policies meeting high standards of care.
  • Prepares and maintains current policies and procedures which meet Medicare, Medicaid, accreditation standards, state and federal laws, etc. and implements; revises concurrently.
  • Displays a willingness to support the policies and procedures and uses appropriate channels for change of such policies.
  • Coordinates interdisciplinary team services, including physician approval of plans for continuity of medical care, and coordinating Hospice’s services with services of other community agencies.
  • Conducts and maintains documentation of care conferences, inservices, and staff meetings.
  • Ensures resident assessments and care plans are developed, reviewed and revised as necessary and according to standards.
  • Observes confidentiality and safeguards all patient related information.
  • Immediately reports accidents, incidents, lost articles, or unusual occurrences to the Administrator.
  • Identifies, investigates and promotes plans of correction in substandard resident care with Administrator.
  • Attends pertinent continuing education programs other than routine in service programs and shares information with staff, participates in community education projects
  • Works with the Administrator in identifying budgetary requirements and determining appropriate use of allocations.
  • Monitors equipment for appropriate use and takes steps to keep misuse to a minimum.
  • Ensures that all necessary supplies and equipment are available.
  • Oversees hospice’s ongoing Quality Assurance/Performance Improvement (QAPI) program.
  • Participates in quality improvement reviews, adverse event reviews, investigations, and evaluations of the nursing services provided, identifying clinical and operational issues needing improvement.
  • Assists in preparation of corrective action plans in preparation for certification and licensure surveys.
  • Assures that nursing services are available 24 hours a day, 7 days a week and that licensed practical nurses work under the direct supervision of a Registered Nurse.
  • Provide 24 hour/day, seven (7) days/week on-call coverage and is normally available during/after operating hours. May designate a qualified temporary replacement if he/she will not be available.
  • Addresses and corrects patient/family concerns. Assures ongoing assessment of patient/family needs.
  • Acts as nurse liaison with staff and other agencies, hospice programs, and individuals with contractual agreements to provide nursing services.
  • Answers correspondence, email and voicemail appropriately and in a timely manner.
  • Ensures that verbal and written communications are understandable, with attention given to feelings, opinions, and attitudes of others.
  • Confirms the standard of current infection control practices are in compliance and being followed, reducing the risk of infection to staff and patients.
  • Review records upon admissions, transfers or discharge for compliance with regulations.
  • Adheres to the HHHR code of ethical behavior and models our core values of respect, passion, and professionalism at all times. Serves as a role model and resource for all colleagues by setting an example of high standards in dress, conduct, cooperation, and job performance.
  • Accepts responsibility for regular attendance and punctuality, fulfills job requirements without regard to time involved.
  • Performs all other duties as assigned.

III. JOB CONDITIONS

  • Physical Requirements:
    • Requires considerable physical effort most of the day including kneeling, squatting, reaching, twisting, climbing, walking, exposure to temperature and humidity changes and maximal assist in lifting and/or transferring of a 100-pound patient.
    • Must possess sight/hearing senses or use appropriate adaptive devices that will enable senses to function at a level required to meet the essential duties of the position
    • Must provide evidence of annual TB test and other state-required tests or examinations.
  • Mental Requirements:
    • Must be able to work independently, make judgments based on assessments and data available and act accordingly.
    • Must be flexible, innovative and possess good interpersonal skills.
    • Must be able to cope with mental and emotional stress and demonstrate emotional stability.
  • Working Conditions:
    • Be able to tolerate exposure to elements including, but not limited to, odors, blood, body fluids and excrements, adverse environmental conditions and hazardous materials.
  • Transportation:
    • Must have a current valid driver’s license, auto liability insurance, and reliable transportation.

IV. ORIENTATION AND TRAINING

Dozoretz Hospice House of Hampton Roads will provide orientation and training, as necessary, to assist the Director in performing the responsibilities listed above. Trainings may be in-house, or outside seminars. Evidence of orientation and training will become part of the personnel file.

V. ACCOUNTABILITY

  • Reports directly to the Administrator
  • Works cooperatively with all members of the Interdisciplinary Team
