Collaborate with providers and lead youth in developing and implementing sensible plans. Monitor safety and behavior management practices while contributing to a positive work environment. Responsibilities include:
- As a Case Manager, you'll play a vital role in improving the lives of up to 10 youth and their families.
- This includes attending Individual Service Plan meetings, collaborating with all team members, and providing support and on-the-job training to treatment parents in behavior management techniques.
- You'll also be responsible for managing crises, monitoring treatment homes, and representing the program at community meetings.
- In addition, you'll maintain positive relationships with funding sources, provide timely and complete record-keeping, and participate in staff and supervisory meetings.
- Other related duties may also be assigned.

Qualifications
- Must be 21 years of age, with valid driver's license.
- Ability to pass all pre-employment checks.
- BSW or BA, BS, in Human Services, Psychology, Sociology or related fields of study.
- At least three years of experience working with at-risk youth
- or 1 year experience, unlicensed Master's level practitioner.
