• Understands own work area professional concepts/standards, regulations, strategies, and operating standards.
• Makes decisions regarding own work approach/priorities and follows direction.
• Always provides CenterWell Home Health gold standard of care.

Required Qualifications

• Minimum required education: LPN licensed in the state appropriate with no disciplinary action
• Experience with clinically assessing, planning, and advocating for patient’s health needs
• Ability to document calls and interaction by typing on a keyboard while speaking to a patient on the phone
• Ability to work independently under general instructions and with a team
• Clear and concise communication
• 1-2 years related work experience in Home Health preferred
• Homecare Homebase (HCHB) system experience preferred

Pay Range

The compensation range below reflects a good faith estimate of starting base pay for full time (40 hours per week) employment at the time of posting. The pay range may be higher or lower based on geographic location and individual pay will vary based on demonstrated job related skills, knowledge, experience, education, certifications, etc.

$40,900 – $56,400 per year
