Primary Purpose:
To work collaboratively with members of the health care team to improve the health and well-being of patients with chronic conditions and/or complex needs. To facilitate communication, coordinate services, address barriers, and promote optimal allocation of resources.

Essential Functions:
Monitors and manages key indicators of population health, particularly with respect to mental health, diabetes and cardiovascular disease.
Identifies patients suitable for care coordination based on criteria such as medical or social complexity, utilization of health services, potential for adverse outcomes, and provider or patient request.
Assists patients in healthcare system navigation with a focus on promoting self-advocacy and expediting care.
Collaborates with Family Health Clinic providers as a patient advocate to achieve optimal patient outcomes.
Cultivates and maintains collaborative relationships within the healthcare referral base and with community services.
Facilitates transitions of patient care between the primary healthcare team and other healthcare services/entities.
Collaborates with HR/Benefits team regarding current ARUP Benefits. Attends in-services sponsored by HR to better understand ARUP benefits. Attends collaboration meetings concerning changes to health plan, offers insights on clinic perspective of daily workings with health plan as a patient advocate.

Physical and Other Requirements:
Communicate: Excellent skills in verbal, non-verbal and written communication with others.
ARUP Policies and Procedures: To conduct self in compliance with all ARUP Policies and Procedu+I3res.
PPE: Biohazard laboratory environment that requires use of personal protective equipment in accordance with CDC and OSHA regulations and company policies.
Sedentary Work: Exerting up to 10 pounds of force occasionally and/or negligible amount of force frequently or constantly to lift, carry, push, pull or otherwise move objects.
Stooping: Bending body downward and forward by bending spine at the waist.
Reaching: Extending hand(s) and arm(s) in any direction.
Mobility: The person in this position needs to occasionally move between work sites and inside the office to access file cabinets, office machinery, etc.
Fine Motor Control: Picking, pinching, typing or otherwise working, primarily with fingers rather than with the whole hand as in handling.
