JOB SUMMARY

The Clinical Access Registered Nurse works under the supervision of the Patient Access Manager. Assists with direction of referrals through guidance to Patient Access staff, nurse navigators, Business Development Executives and Clinical Liaisons through guidance to appropriate access within the continuum of care. Collaborative work and communication with Patient Access Specialists and Patient Access Manager as well as thorough review of medical records to ensure clinical details are addressed and eligibility is determined for appropriate LCFL program. Monitors conversion ratio as well as time frame between referral to admission processes to ensure a seamless patient referral and admission process is attained. Staff in this key position must have the clinical knowledge and critical thinking ability to effectively plan and provide coordination of patient care. The Clinical Access RN must also possess multi-tasking skills as well as the ability to collaborate and cooperate with colleagues. Directs all activities in a highly professional manner, demonstrates behavior that exemplifies LCFL mission and values, and applies principles of leadership and professional standards to all job responsibilities. Performs other duties as assigned.

PRIMARY JOB DUTIES

1. Oversees all calls coming into The Patient Access Department. Treat all calls coming into the agency as a referral until proven otherwise. Acts as an in-office clinical resource with referrals for patient care. 2. Ensures appropriate documentation is obtained and thoroughly reviewed to allow confirmation of care eligibility, and identify hospice patients requiring complex case management. 3. Follows up on referrals not admitted analyzing information to determine proactive steps to reduce non-admits. Create process upon non-admission and/or discharge with referral to other LCFL programs as appropriate for patient. 4. Reviews records to ensure appropriate documentation is obtained to demonstrate eligibility and meet documentation requirements for admission to the appropriate program. Works in conjunction with the Palliative Care RN to navigate patient care to the appropriate service line. Communicates with referral source to obtain additional documentation when required. 5. Appropriately documents and communicates all required information in accordance with agency policies and procedures, accreditation, regulation, and legal requirements. 6. Communicates with other agency departments to coordinate consistent messaging and enhance customer service to all possible customers. 7. Organizes and performs work effectively and efficiently. 8. Maintains and adjusts schedule to enhance program performance. 9. Demonstrates positive interpersonal relations in dealing with all members of the team. 10. Maintains and promotes customer satisfaction. 11. Effectively demonstrates the mission, vision, and values of Lower Cape Fear LifeCare on a daily basis.

JOB SPECIFICATIONS

1. Education: Preferred Hospice clinical experience. (Graduate of an accredited school of nursing, either through an RN, LPN and/or BSN program)

2. Licensure / Certifications: Current license to practice professional nursing in the State of North Carolina / South Carolina.

3. Experience: Two years clinical experience. Prefer hospice or home care, admission, experience in marketing, or hospital liaison nurse experience, but not required.

4. Essential Technical / Motor Skills: Ability to speak clearly to communicate with patients, families, physicians, and staff. Good command of the English language. Computer experience helpful.

5. Interpersonal Skills: Ability to develop positive interaction with patients, families, physicians, and staff and other health professionals in order to effectively communicate and educate about hospice services and end-of-life care. Strong verbal and written communication skills. Highly self-motivated and self-disciplined. Flexible.

6. Essential Physical Requirements: Ability to move freely. Ability to work a full week and manage stressful situations. Ability to lift and/or maneuver objects weighing at least 45 pounds or less in the assessment and implementation of patient care.

7. Essential Mental Abilities: Ability to define problems, collect data, establish facts and draw valid conclusions. Requires higher level of mental faculties accompanied by short and long-term memory.

8. Essential Sensory Requirements: Touch, sight, hearing, ability to organize thoughts, facts and ideas. Ability to translate the written word.

9. Exposure to Hazards: May be exposure to blood and body fluids, infectious diseases, needle puncture wounds. May encounter patients and other situations, which present a potential threat to personal safety.

10. Other - Hours of Work: Flexibility with schedule required to meet the needs of the department.
