Clinic RN Coordinator - EHR Inbasket Specialist
POSITION SUMMARY
The Clinic RN Coordinator will support primary care providers and care teams with electronic health record (EHR) tasks, medical message management, panel management, and coordination of care needs. The RN will respond to all EHR medical messages by offering clinical guidance, triage, care coordination, and patient education prior to heightening to provider level, if indicated. This role will work collaboratively with clinic leadership and our data integration team to optimize EHR message management and electronic platforms. Responsible for providing professional nursing care for patients including assessment, treatment, care planning, and medical care evaluation by following established standards and practices. This role works collaboratively with others to restore health and promote wellness through coordinating comprehensive nursing care for patients of all ages, gender, economic and social status, and backgrounds.
POSITION QUALIFICATIONS
Education and Experience
Graduate of an accredited School of Nursing with an Associate Degree or higher.
Minimum of one-year professional nursing experience; clinic experience strongly preferred.
Experience with electronic health record (Epic) strongly preferred.
License/Certificates
Current Minnesota RN license.
BLS is required
ESSENTIAL DUTIES & RESPONSIBILITIES
Monitor primary care providers' electronic health record inbasket, prioritize inquires, and delegate tasks accordingly
Serve as primary contact for responding to telephone, medical messages, and/or other tasks sent through electronic health record that may require nursing intervention.
Provide timely care coordination to patients with health concerns or questions that may arrive via face-face, telephone, and/or through the electronic health record.
Utilize nursing process and approved decision-making protocols to assess the health care need and urgency of patient inquires, based on reported symptoms and history.
Independently address concerns/questions within RN's scope of practice; involving the provider(s) either emergently or as needed for issues requiring their involvement.
Document all contacts and inquiries thoroughly and accurately in the electronic health record.
Demonstrates clinical competence in providing direct patient care, education, and assisting in coordination of care.
Educates patients and families about health status, health maintenance, and management of acute and chronic conditions.
Support chronic disease and panel management by utilizing patient registries to ensure optimal care is achieved.
Documents patient assessment and intervention data using established medical record forms/automated systems and documentation practices.
Work collaboratively with physicians, advanced practice providers and other multidisciplinary team members internal and external to Riverwood Healthcare Center.
Participate in daily huddles to support team-based care, coverage, and patient outreach needs.
Provide care coordination to patients through clinical assessments, developing individual care plans, health coaching, and evaluating the effectiveness of interdisciplinary care team support.
In collaboration with clinic leadership and data integration team, ensure efficient and effective documentation practices and workflows are in place to optimize clinical practice needs.
Review incoming prescription requests and processes these using established nursing practices and adopted protocols in an efficient and timely fashion.
Assist with quality improvement and population health outreach activities as assigned.
Assist with policy, protocol, and standing order development as assigned.
