Job Summary:
Provides primary clinical care management and coordination of care for assigned patients under the direction and clinical supervision of the medical director or designee for the program. Services provided may include coordination of program resources, coordination of quality improvement initiatives, and coordination of the continuum of care to these patient populations as well as to any patient in the assigned clinic.
EXPERIENCE DESCRIPTION:
Two to three years in health care experience is required.
EDUCATION DESCRIPTION:
High School degree required. Bachelor's degree in business administration or healthcare administration is preferred.
LICENSURE DESCRIPTION:
American Heart Association (AHA) Basic Life Support for the Healthcare Provider (BLS) or an AHA approved equivalent is optional.
