CCMH is hiring a Client Medication Coordinator for our Cornerstone Residential Facility. The position is 40 hours, 5 days a week, from 8am to 5pm with an hour-long lunch break. There are opportunities for overtime! We are looking for someone that enjoys wearing many hats and thrives in a team environment.

SUMMARY: The Client Medication Coordinator is responsible for medication oversight and training at assigned residential facilities. Essential duties are determined/assigned per location, position and/or department.


ESSENTIAL DUTIES AND RESPONSIBILITIES:
• Administer medications/treatments per MAR (Medication Administration Record)
• Monitor medication changes as directed by physician orders
• Monitor all schedule II medications per agency policy
• Track/oversee all medication ordering/supply to ensure medications are on site without interruption to medication schedule
• Process all incoming prescriptions/medications/treatment orders for new clients
• Establish a PCP for incoming residents as needed
• Maintain accurate/updated Client Medication records
• Create/review/correct all MAR’s as needed per physicians’ orders/prescriptions and on monthly basis to ensure accuracy
• Assist in staff training as it relates to medication management and documentation
• Maintain positive relationships with community partners
• Assist clients with understanding/identifying medications
• Oversee community client mediation cassette refills and weekly residential medication group
• Schedule/transport clients for medical & prescriber appointments as needed
• Responsible for preparing/maintaining weekly client schedules
• Proper disposal of discontinued medications as needed
• Other duties as assigned

EDUCATION and/or EXPERIENCE: High school diploma or equivalent required; Associates or Medical Assist training preferred; Two years medication management required; Experience working with addictions and/SPMI population highly preferred. Combination of education/experience that demonstrates the ability to perform the duties outlined for this position may be considered.
