Job description

Responsibilities of this position include:

Must Live in San Diego, CA

Evaluate and validate all prospects/client leads.

Manage all aspects of the client’s home care assessment process. Must complete assessments in person.

Complete Client home care assessment utilizing appropriate forms and Client Assessment Packages;

Communicating completed written assessments to Staffing Manager in a timely manner.

Establish and maintain positive relationships with customers and referral sources, responding to customer requests and concerns in a timely manner.

Ensure that quality service is provided to meet Client Care goals and needs.

Maintain comprehensive working knowledge of community resources and home care assistance (e.g., Aid and Attendance; Respite Care; Long Term Care Insurance, etc.) to offer information and guidance to prospects and current clients.

Make referrals to other services when client conditions or situations require the service of other professional disciplines or the products or services of outside companies or agencies.

Counsels and guides individuals and families towards self-help in recognition and solution of physical, emotional and environmental

Participate in needs assessment process to identify and develop training, orientation, and performance evaluation content.

Provide feedback to Office Manager to assist with staff performance evaluation process.

Address and attempt to resolve client issues and incidence reports to assist Staffing Coordinator and On-Call Staffing personnel, as needed.

Actively participates in Interdisciplinary Team Meetings.

Performs periodic on-site evaluations of affiliated Home Care Aides (Caregivers) in relations to Client care goals/needs and reports findings and recommendations to Office Manager.

Performs other duties as assigned.

Ensure seamless transition of clients from hospital/rehab/skilled nursing facilities into non-medical in-home health services.

Competitive base pay plus commissions.

Job Type: Full-time

Pay: $65,000.00 - $75,000.00 per year
