The Chronic Care Management Nurse works as part of the clinic provider team, assists the provision of primary health care, provides direct patient services, and performs specifically designated independent procedures. The Chronic Care Nurse will plan and coordinate self-Care Management as part of a multidisciplinary team. The Chronic Care Management Nurse will work with the provider and other clinical staff to develop a comprehensive service delivery system aimed at meeting the needs of the patients/clients. Also will be responsible for monitoring remote blood pressure devices for those enrolled in program. Applicants must have a current RN license and CPR. Will consider LPN for position.
