About the Consortium

The West Philadelphia Community Mental Health Consortium, Inc. (Consortium) is a nonprofit (501(c)(3) Community Mental Health Center (CMHC) established in 1967 as one of Philadelphia’s twelve CMHCs. Throughout our history, we have pioneered providing behavioral health services with ‘compassion first’ priorities coupled with excellence in Mental Health treatments and collaborative understanding. For over five decades, we have helped more than 250,000 people throughout the tri-state area overcome the challenges of mental illness and substance abuse. We have been the home of many firsts, from the first methadone clinic to the first clinic providing integrated care. We have a proud and diverse history that is driven by our community. Located in West Philadelphia, the Consortium is the premier provider of Supports Coordination, Outpatient Mental Health, Addictions, Case Management, Eco-systemic Family Therapy (ESFT) and Functional Family Therapy (FFT) for all ages in West and Southwest Philadelphia, serving approximately 4,000 individuals in 2022. Utilizing Evidence Based Practices (EBP) in all programs, the Consortium is accredited by CARF for outpatient drug and alcohol services and has received the EBP Designation through Community Behavioral Health’s Evidence-based Practice and Innovation Center (EPIC) for both Cognitive Behavioral Therapy (CBT) and Prolonged Exposure (PE) for Post-Traumatic Stress Disorder (PTSD). Most recently, the Consortium’s Integrated Care Program was recognized as “Most Innovative Program” by the City of Philadelphia.

What is Expected in this ROLE

  • Experience in Childrens Outpatient setting and provide counseling and case management services to youth with psychiatric and/or co-occurring disorders.
  • Conduct assessments, individual, family and/or group counseling sessions and outreach to individuals.
  • Responsible for meeting the required hours of direct service on a weekly basis.
  • Responsible for being on time and preparing for scheduled appointments.
  • Develop and maintain complete clinical records including assessments, treatment plans, progress notes, case conferences, discharges and other clinical and administrative documents in a timely manner. Ensures charts adhere to established chart sequence, date order, and that all documents are present.
  • Complete assessments, treatment plans, progress notes, discharges, and other clinical and administrative documents in accordance with agency, managed care, and licensing requirements and standards. Regularly checks to ensure that content, signatures, credentials, dates, times, service types and legibility meet standards.
  • Submit status of all scheduled services with corresponding documentation to designated staff within 24 hours of each event.
  • Provides clinical assessments and crisis management in consultation with supervisor, psychiatrist and/or treatment team, as required. Responsible for monitoring individuals in crisis on the unit. Assists with coordinating hospitalization procedures, if necessary.
  • Follow unit procedures to schedule appointments with staff psychiatrist as appropriate.
  • Attend and participates in case conferences and staff meetings. Utilizes input from those meetings to update treatment plans and/or incorporate treatment strategies discussed.
  • Attend bi-weekly and bi-weekly supervision meetings. Comes prepared with an agenda to discuss items such as successes, concerns, ideas or follow-up items.
  • Complete training and seminars per departmental, agency, managed care and regulatory requirements.
  • Makes referrals to other Consortium units, outside agencies, or support networks.
  • Follows-up on referrals to ensure continuity of care. Follows up on referrals related to discharge within 30 days. Ensures all referrals and follow-up efforts are documented.
  • Manage caseload to assess individual progress, reviewing appropriateness of and interest in service provided.
  • Complete Utilization Review.

  • Crisis prevention/intervention orientated.
  • A valid driver’s license matching the residential state with a clean driving record is required.
