Children's Case Manager: JFCS is seeking enthusiastic and empathetic Case Managers to assist children who may be receiving behavioral-health services on the path to a more self-sufficient lifestyle and/or family reunification. The Case Manager will also provide guidance and support to the families/caregivers of these youth as needed.
- Positions available in Glendale, Phoenix, & West Valley.
Duties include:
- Provides Case Management services to assigned clients.
- Follows required guidelines for monthly contact, quarterly home/community visits, attending medication checks, etc.
- Assists clients in finding appropriate services and works with children to decrease behaviors and improve stability in the community & home.
- Responsible for coordinating and facilitating Child & Family Team (CFT) meetings.
- Collaborates with clinical team to ensure that Integrated Service Plans, CFT meetings, and all required documentation are completed accurately and in a timely manner.
- Collaborates and coordinates services with other involved agencies, such as the Department of Child Safety (DCS), Probation/Parole Departments, Division of Developmental Disabilities (DDD), schools, and other providers.
- Completes referrals, prior authorizations, and other paperwork as required to secure appropriate services for clients.
- Will inquire about the client’s health & wellness needs to assist in coordinating primary care or other supportive services with appropriate JFCS staff or external parties to ensure the client’s whole health is addressed.
Qualifications:
-Meets one of the following education/experience requirements:
- High School Diploma/GED required and 4 years of Behavioral Health experience.
- A.A. Degree and two years of Behavioral Health experience
- Bachelor’s Degree in Social Work, Psychology, or related field.
-Additional qualifications:
- Level 1 Fingerprint Clearance Card without Restriction(s)
- Meet Agency requirements for MVR (Motor Vehicle Record) report
