Overview

Join the CSHN team as a Care Consultant to provide support cross multiple sectors. Requires professional-level knowledge of human development and the system of care, on-going collaboration and partnership with families and community providers working with the Special Healthcare needs population. Goals include ensuring families are connected to appropriate resources; acting as resource for providers; identifying and closing systems gaps; assisting and informing regional program development; and engaging in complex interactions with public and private organizations. Work will focus on improving access for the CSHN population. Effective networking, communication, and problem-solving skills are essential for this work, as is the ability to adapt skillset to different regions of the state. Active contribution and participation on regional teams and quality improvement projects.

AHS BACKGROUND CHECKS: Candidates must pass any level of background investigation applicable to the position. Environmental Factors

Duties are performed primarily in a standard office setting, but require frequent travel to community sites, for which private means of transportation must be available. Public speaking and attendance at public meetings may be required. Some work outside of normal office hours may be required.

Minimum Qualifications

Bachelor's degree AND four (4) years or more of professional level experience in administration, planning, or supervision in a public health, health care or human services field. Including two (2) years of experience in public health.


OR


Master's degree in public health, public administration or a health field AND two (2) years or more of professional level experience in administration, planning, or supervision in a public health, health care or human services field.
