GENERAL STATEMENT OF DUTIES
A Social Worker in this position will primarily act as a permanency Social Worker for youth in the Extended Foster Care 18-21 Program and LINKS Coordinator for Caldwell County Department of Social Services.
EXAMPLES OF DUTIES PERFORMED
Extended Foster Care 18-21
- Interpreting and implementing policy
- Managing caseload of youth enrolled in the program
- Engagement in mandated court processes
- Conducting required home and community visits
- Helping youth to make connections with family and/or community
LINKS
- Understanding and implementing policy and practice
- Coordinating activities, which may include partnering with other local department of social services
- Conducting in-house trainings for youth in the LINKS Program
- Tracking LINKS expenses
- Reaching out and partnering with permanency Social Workers to assist with youth on their caseloads to participate in the LINKS Program
- Tracking LINKS eligible children and youth
- Completing credit checks for LINKS children and youth
- Ensuring completions of Casey Life Skills Assessments
Recruitment
- Attending events to engage in foster and adoptive family recruitment efforts
- Assisting with foster parent training
Other duties, as assigned
RECRUITMENT STANDARDS
Knowledge, Skills and Abilities
Basic computer skills, organizational skills, ability to prioritize and multi-task, effective communication skills (communicating with clients and families as well as community based providers, physician offices, etc). This is in addition to social work skills and experience including assessment and counseling, mediation skills, medical terminology and knowledge of nursing facilities.
MINIMUM EDUCATION AND EXPERIENCE
BSW from an accredited School of Social Work in an accredited college or university or 4-year degree in a human services field plus one year directly related experience, or 4-year college or university degree plus two years of directly related experience. Directly related experience is defined as, “human services experience in the areas of case management, assessment and referral, supportive counseling, intervention, psycho-social therapy and treatment planning.”
