Caregiver-Personal Care (CNA) (PCA)
OBJECTIVE: To provide professional care giving services and to help clients maintain their
independence and to assist them with the activities of daily living and household and family support
services. The job Description outlines your primary responsibilities as a caregiver. This description
is not meant to be an exhaustive list of services, as the exact duties to be performed for each
assignment will vary. Prior to beginning a new assignment, Quality First Healthcare Services will
provided you with background information about your client and review any special needs this
client may have.
Time: 4 pm to 10 am Monday thru Sunday is the shift
Primary Responsibilities:
Companionship conversation
Light Housekeeping
Household management
Walking assistance
Eating assistance
Errands and Transportation
Meal planning and preparation
Laundry linen changing
Bathing assistance
Dressing assistance
Toileting
Medication reminders
Monitor Alzheimer’s Dementia
clients
Assist client to ad from bed
Assist client throughout the day
Other Requirements:
Pass recurring 90-day skills assessment and client review.
Sign off on tasks in Web Author
Clock in and out on EVV Web-Author
Report any significant client changes to the office immediately
Follow all policies and procedures as outlined in the Quality First Healthcare Services
Employee Handbook and Orientation Manual
Report any TB Hepatitis exposure immediately
Qualifications:
Ability to treat clients with dignity and respect
Ability to read, write and speak English at an average adult level
Must have a reliable vehicle and keep it properly maintained
Valid driver’s license with no serious driving violations
Pass a criminal background check and include statements of history of no abuse/neglect
Documentation of TB screening

Must have a current CPR and First Aid Certification
GA-Registered CNA
Completion of the NLN exam on-line and assessment of competency for services to be
performed, or completion of a provided 40-hour training curriculum and assessment of
competence. (PCA Test)
