JOB SUMMARY
- As a Certified Nursing Assistant (CNA) at Horizon Home Health and Hospice, you will be a valued member of our home care team.
- You will work under the supervision of a registered nurse or therapist, providing various personal care services to meet the patient's needs.
- Your responsibilities will include observing patients, reporting observations, documenting care performed, and promoting quality, continuity, and safety of patient care.
- You will also be part of our interdisciplinary team, collaborating with other healthcare professionals to ensure comprehensive patient care.
DUTIES & RESPONSIBILITIES
- Provide personal care services such as baths, back rubs, oral hygiene, shampoos, changing bed linen, assisting with dressing and undressing, nail and skin care, and toileting activities.
- Plan and prepare nutritious meals for patients.
- Assist in feeding patients, if necessary.
- Take and record vital signs, including temperature, pulse, respiration, and blood pressure.
- Assist in ambulation and exercise according to the plan of care.
- Perform range of motion and other simple procedures as ordered.
- Assist patients in self-administration of medication.
- Keep patient's living area clean and orderly.
- Report on patient's condition and significant changes to the assigned nurse.
- Adhere to documentation and care procedures, as well as personal and professional conduct standards.
- Attend in-service training annually to enhance your skills and knowledge.
- Participate in scheduled Case Conference meetings as part of the Interdisciplinary Group (IDG).
JOB REQUIREMENTS
- Must have successfully completed a training and competency evaluation program approved by the respective state and listed in good standing in the state nurse registry.
- At least 18 years of age.
- Ability to read and follow written instructions and document care given.
- Self-directing with the ability to work with little direct supervision.
- Empathy for the needs of the ill, injured, frail, and impaired.
- Possess and maintain current CPR certification.
- Demonstrate tact, patience, and good personal hygiene.
- Licensed driver with an insured automobile in good working order.
