Job Title: Certified Nursing Assistant (CNA); Palo Alto VA Medical Center - Menlo Park, CA
Grit Government Solutions is looking for a qualified Certified Nursing Assistant (CNA) located at the Palo Alto Veterans Assistance Medical Center-Menlo Park
Unit: Extended Care or CLC (Community Living Center)
Setting: Long-Term- Care Schedule: M-F 3pm - Midnight must cover every other weekend
Requirements:
· CNA License (unrestricted)
· BLS (AHA)
· A minimum of 2 years recent CNA experience
Specific Duties & Responsibilities:
· Gather information regarding the health care needs of the patient/resident and assists the RN with the development with a plan of care to meet interdisciplinary goals
· Notify RN of any significant changes in the patient’s/resident’s health condition
· Collect and document pertinent data into the electronic patient/resident medical record
· Carry out the plan of care by performing assigned tasks
· Complete nursing documentation within required time frames
· Identify and act on predictable needs of the patient/resident
· Meet the emotional and spiritual needs of patient/residents and/or families
· Prioritize and complete tasks within the CNA’s assigned shift
· Report incidents of verbal or physical abuse immediately to the Nurse Manager of the unit they are assigned; or designated off-shift nursing leadership
Assist with nutrition feeding and set up trays
· Assist with routine urinary and bowel elimination including emptying catheters, emptying colostomies
· Assist with mobility including transfers, transfer devices, and ambulation
· Clean and stock patient’s/resident’s room
· Collaborate with patient/residents, families, other health care disciplines, and nursing staff to formulate treatment plans and outcome criteria
· Educate patient/residents and/or families as instructed by RN
· Measure and/or document vital signs including Pain (the 5th Vital Sign), pulse oximetry, etc.
· Measure and/or document intake and output for urine and drains
· Provide comfort measures including repositioning, bathing, activities of daily living, range of motion, and hygiene (shaving, brushing teeth, dressing)
· Obtain and deliver urine specimens to the lab
· Activate the Emergency Medical Response as indicated based on patient presentation or change in condition
· Respond and assist in an emergency situation as instructed
· Screen patient/residents for history, complaints, and vitals
· Perform Continuous Protective Observation (CPO) duties to maintain veteran safety
Job Type: Full-time
Pay: $22.25 per hour
Expected hours: 40 per week
