Horizon is accepting applications for a Certified Nursing Assistant. Our Healthcare Centers specialize in short-term rehabilitation, as well as providing excellent care to our long-term- residents.

Basic Qualification

Education/Training: A high school diploma or equivalent; successful completion of a North Carolina State state-approved Nursing Assistant Certification course or other education that meets state certification requirements. Must hold current Nursing Assistant Certification in North Carolina State 

Skills: Moderate reading, writing, grammar, and mathematics skills; strong people skills and communicative skills; ability to remain calm and courteous in stressful situations; auditory and visual skills; ability to bend, stoop, sit, stand, reach, and lift items weighing 50 pounds or less.

Experience: A minimum of three months prior Nursing Assistant experience in long-term care is preferred.

  • Protect Resident Rights and ensure an atmosphere that allows for the privacy, dignity, and well-being of all residents in a safe, secure environment.
  • Support, coordinate with, and implement specific procedures and programs for Safety, including universal precautions and safe work practices, established fire/safety/ disaster plans, risk management and security, report and/or correct unsafe working conditions, equipment repair, and maintenance needs
  • Confidentiality of all data, including resident, employee, and operations data.
  • Support and participate in common teamwork: Cooperates and works together with all co-workers; plans and completes job duties with minimal supervision direction, including appropriate judgment.
  • Uses tactful, appropriate communication in sensitive and emotional situations.
  • Follow up as appropriate with supervisor, co-workers, or residents regarding reported complaints, problems, and concerns.
  • Promotes customer service with residents, family members, and guests.
  • Adheres to requirements for in-service training; attendance, uniform, department and dress code policies, including personal hygiene, and completes other work duties as assigned.

Essential Duties Provide quality nursing care to residents while maintaining an environment that promotes resident rights, dignity, freedom of choice, and individuality as illustrated by the following:

  • Answer residents’ call lights promptly and courteously, and ensure the call cord is within residents’ reach upon exiting the room.
  • Attend to the individual needs of residents which may include assistance with grooming, bathing, oral hygiene, feeding, incontinence care, toileting, colostomy care, prosthetic appliances, transferring, ambulation, communication, or other needs in keeping with the individual’s care plan.
  • Complete CNA records daily, and document 100% of care provided into charting software in keeping with company policies.
  • Encourage residents to attain their highest practical physical, mental, and psychosocial well-being.
  • Become knowledgeable of the individualized care plan for residents and provide support to the residents according to the care plan. Provide pertinent information to the charge nurse or other care planning staff with specific information, such as changes in condition or behavior, based on observation of the resident’s needs and preferences.
  • Maintain the comfort, privacy, and dignity of each resident in the delivery of services to them. Interact with residents in a manner that displays warmth and promotes a caring environment.
  • Fully understand all aspects of residents’ rights including the right to be free of restraints and free of abuse. Responsible for promptly reporting to the charge nurse or administrative staff of incidents of abuse, neglect, misappropriation, or violation of residents’ rights.
  • Assist in maintaining a safe, neat, and clean environment; report environmental hazards to the supervisor such as broken equipment, tripping hazards, etc.
  • Protect the personal belongings of each resident including eyeglasses, dentures, hearing aids, furnishings, jewelry, clothing, memorabilia, etc. Promptly report missing items according to established policy and participate in efforts to locate missing items.
  • Provide care that maintains each resident’s skin integrity to prevent pressure ulcers, skin tears, and other damage by changing incontinent residents; turning or repositioning immobile residents, and applying moisturizers to fragile skin as directed by the care plan.
  • Perform various tasks assigned by the supervisor including vital signs, resident weights, applying creams/ointments, collecting specimens, etc.
  • Lift, move, and transport residents according to care plan, using proper body mechanics for lifting devices for accident prevention.
  • Practice appropriately using supplies and linen and follow established procedures for billable resident items.
  • Perform all job responsibilities in accordance with prescribed safety and infection control procedures including thorough hand washing, use of disposable gloves where indicated, and proper disposal of soiled materials.
  • Support an atmosphere that promotes a homelike environment; for example, knocking prior to entering resident rooms, maintaining quiet in hallways and during shift changes, etc.
  • Provide prompt customer service to residents, families, visitors, co-workers, etc. within given time frames and within established policy.

Performs other duties as assigned by supervisor which are supportive in nature to the essential functions of the job, but which may be altered or redesigned depending upon individual circumstances.

  • Meet ongoing requirements to maintain current Certified Nursing Assistant certification.

Physical Demands of the Job: Frequent tasks involving walking throughout the shift. Distances typically less than 200 feet with occasional walking greater distance, walking is on even surfaces.

  • Occasional sitting during duties, the majority of which will be at the nurse’s station.
  • Occasional standing does not need to be static.
  • Occasional squatting and/or bending, necessary mostly during medication pass or resident assessment.
  •  
  • Repetitive movements require following proper body mechanics. Notify the supervisor of any concerns. Fine repetitive motor skills will be used frequently; gross repetitive motor skills will be used occasionally.
  • Occasionally push/pull up to 25 pounds of force.
  • Occasional twisting, seldom climbing or using stairs, being able to stand/walk for extended periods.
  • Frequently reach from shoulder height to knees; seldom reach above shoulders or below knees.
  • During the Covid-19 outbreak, N-95 masks are to be worn during the duration of your shift.
  • When a facility is not in an active outbreak staff are to wear surgical masks, KN-90/95s unless on observation to which N-95s are still mandated.

Conditions of the Environment. Controlled temperature, primarily indoor, well-lit, maintained flooring, smoke-free, and occasional odors that dissipate. The noise level is maintained from quiet to moderate. Areas of Moisture exposure during the bathing process. Potential exposure of COVID-19 and mutated strains during the Pandemic.
