CENTRALIZED ADMISSIONS DEPARTMENT COORDINATOR
Position Description:
The Centralized Admissions Coordinator will navigate referrals through the admission approval process in a consistent and timely manner for multiple Symphony skilled nursing facilities. This position reports directly to the Centralized Admissions Director.
DUTIES:
- Coordinating the resident referral and approval process in an efficient manner following an established clinical skills inventory.
- Maintaining data entry in the CRM for quality trends analysis.
- Working closely with hospital liaisons and the centers with referrals, focused on hitting/exceeding daily admissions targets.
- Establishing & maintaining relationships with local hospitals including emergency departments, physicians, home care agencies & other community organizations focused on growing referrals and admissions year over year.
- Collaborates with facility administrators and directors of nursing on quality transitions of care.
- Understands the admissions process including policy, procedures, assessments & paperwork requirements.
- Maintains knowledge of regulations governing Medicare/Medicaid/Managed Care reimbursement processes.
- Other duties as assigned.
REQUIREMENTS:
- Previous experience working in a long-term care facility is strongly preferred.
- Strong focus on customer service and excellent communication (verbal and written).
- Strong assessment, organizational and data entry skills.
- Sense of urgency with the ability to multitask in a fast-paced environment.
- Demonstrates leadership skills, self-initiative and personal accountability.
- Solid computer skills; working knowledge of MS Office.
- Willingness to work “restaurant/retail” hours which will include some evenings, weekends and holidays. Symphony believes in a quality work life balance so any extra time involved will be recognized.
- High school degree or equivalent. College degree preferred.
