***GRANT-FUNDED***  This is a grant-funded position.  The position's continuance may be dependent upon on-going grant funding.

GENERAL RESPONSIBILITY:

 

To assess and enroll individuals who are eligible for participation in Goodwill's Pathway Home Program.

General Responsibilities

 

To provide Case Management for individuals from Pre/Post-Release correctional institutions including; Education, Training, Employment, and follow up Services to Pathway Home program participants.

 

To enroll, train, assess and mentor participants, and prepare them for educational advancements, employment and ensure program participants are progressing towards the goals identified in the Individual Development Plan (IDP).

 

To represent Program Manager in absentia at community events, partner meetings, general Pathway Home and other events as assigned.

 

To meet/exceed DOL grant outcomes for the Pathway Home program.

 

 

Essential Duties and Responsibilities

 

1.     Maintains accurate and complete records for all program participants for the purpose of case files and reporting requirements.

2.     Completes intake and orientation processes with participants.

3.     Performs all participant pre-enrollment screenings to ensure compliance with program acceptance criteria.

4.     Schedules, coordinates and facilitates all program enrollments and orientations. These may occur pre-release and post- release.

5.     Identifies participant program track to include; education, employment or training services.

6.     Assist with training new staff on case management best practices, ETO, Pathway Home software and file maintenance.

7.     Ensure all required paperwork is complete for each case manager's new participants prior to enrollment.

8.     Identifies jobs in the community and places program participants into gainful employment.

9.     Review SLED and National/Federal background reports for appropriate services or program referrals.

10.  Collects, organizes, and analyzes information about program participants through records, assessments, interviews, and professional sources to appraise their interests, aptitudes, and abilities for vocational planning.

11.  Conducts intake, assessments, and orientation for the Pathway Home Program.

12.  Understands and maintains current knowledge of labor market and workforce information.

13.  Provides occupational, educational, legal and economic information to aid program participants in establishing realistic vocational and educational objectives.

14.  Develops individualized development plans (IDP's) for program participants that include appropriate goals and objectives.

15.  Tracks and records each program participant's progress to ensure continual progression toward individual goals.

16.  Assists program participants to understand and overcome barriers to employment and determines the support services necessary to overcome these barriers.

17.  Provides pre-employment, skill-based, and job development training to program participants.

18.  Develops effective relationships with businesses and industries in order to educate and advise participants regarding program services.  Contacts prospective employers to determine their needs and explain placement services.

19.  Responsible for maintaining program participant database and software associated with case management activities efficiently and in accordance with Dept. of Labor and GIUMSC policies and procedures.

20.  Communicates professionally with supervisors, co-workers, program participants, community agencies, and other stakeholders involved in service provision.

21.  Facilitates training classes and presentations.

22.  Conducts work in safe and responsible manner and complies with all safety standards.

23.  Responsible for awareness of and adherence to all company, confidentiality and safety policies and procedures.

24.  Reports all program, office and staffing issues to Team Lead and/or Mission Manager.

25.  Performs all other duties as may be assigned.

Education: Bachelor's Degree from an accredited college and one to two years related experience; or combination of education and experience. Preferred experience in working with the ex-offender population.

 

Knowledge, Skills and Abilities:

  • Proficient and skilled in MS Office (Excel and Word) and Outlook
  • Excellent oral and written communication skills
  • Knowledge of social service, labor market information and community agency practices
  • Ability to network and develop relationships with agencies and employers
  • Have a working knowledge and familiarity with basic ADA regulations
  • Ability to:
  1. Read, write, speak and understand English
  2. Give and carry out instructions in verbal and written format
  3. Communicate effectively and efficiently
  4. Interact and maintain effective working relationships with individuals of varying social and cultural backgrounds and people with disabilities and disadvantages
  5. Conduct presentations in a professional manner

  8. Maintain confidentiality
  9. Handle multiple tasks and meet deadlines

 

Minimum Qualifications: To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. Must be able to drive, and must possess a valid South Carolina driver's license, liability insurance, dependable vehicle, clean driving record. Ability to successfully complete a criminal background check and drug/alcohol test.

 

Physical Requirements: Daily requirement may include balancing, reaching, talking, hearing, seeing, standing, stooping, kneeling, handling and lifting, carrying, pushing and pulling, May be exposed to dust and odors. May be required to do limited work outdoors.

 

Safety & Security:  Responsible for following all safety, security, and money handling procedures, as directed by GIUMSC policy.  

 

Work Hours: 8:00 am - 5:00 pm Monday - Friday; however, there will be times that position requires work in the evening to meet the professional requirements of the job. Ability to work inside a correctional institution and delivery services.

 

Work Attire: Business attire; however, professional attire when appropriate.  Dress is reflective of GIUMSC's positive image in the community.  Each associate should present a fashionable, neat and clean appearance at all times.
