Summary
At an intermediate level, assess client needs and assist with access to care as needed.
This may also include providing case management services; determining eligibility for local, state and federal programs; making referrals to community agencies; serving as liaison between client and health service provider; and ensuring diagnostic services and reports are receive in timely manner.
Duties include providing medical health and social services in a coordinated manner.
Grant funded.
Work Schedule: M-F 8:00am - 5:00pm

Essential Duties and Responsibilities
- Assesses clients' needs and provides medical and social case management
- Provides information and refers clients to community agencies as needed
- Provides short term crisis intervention counseling to HIV+ clients
- Documents intervention on client chart; maintains confidentiality
- Determines eligibility for local, state, federal programs and assists clients with applications
- Participates in weekly and monthly case conferences with PMC staff and other AIDS service organizations
- Develops and maintains a network of professional contacts which may include therapists, caseworkers, law enforcement agencies, state or federal agencies, and community based service agencies for the purpose of obtaining information and back up service for clients
- Provides discharge planning to HIV+ inmates at Tarrant County Corrections who are soon to be released
- Performs in-house assessment of clients' situations as needed
- Documents appropriate information in client reports appropriate information to primary healthcare provider(s). Maintains confidentiality of client information as outlined in HIPAA guidelines
- Serves on Department committees including team for bioterrorism preparedness and response capacities
- Performs all other related duties involved in the operation of the Public Health Department as assigned or required
Minimum Requirements

TO APPLY, must have a Bachelor's degree or higher in Social Work or Nursing. Master's degree preferred.
TO APPLY, must have One (1) full-time year of social work experience required; experience working with HIV/AIDS programs preferred.
TO APPLY, must possess current Texas Social Work license or RN licensure.
NOTE: MUST PROVIDE LICENSING AGENCY AND LICENSE NUMBER IN APPLICATION IN THE "License" FIELD OR RISK DISQUALIFICATION.
Spreadsheet and/or word processing software experience preferred, such as Microsoft Office Suite.
Good interviewing, listening and other verbal and written communication skills.
Experience in reading and analyzing case records.
Required to complete annual training hours to maintain professional certification or licenses and required to stay abreast of new procedures and technologies involved in the field.
If hired, must provide proof of educational attainment
at New Hire Processing or during the promotional process.

While performing the duties of this position, the incumbent is regularly required to sit, stand, walk, bend, lift, stoop, push and pull. May deal with resistant clients refusing recommended health care.
