Job Responsibilities
This posting seeks to fill seven (7) positions with one (1) Lead Coordinator.
About Our Agency
The South Carolina Department of Disabilities and Special Needs (DDSN) is the state agency that plans, develops, coordinates, and funds services for South Carolinians with the severe, lifelong disabilities of:
  • intellectual disability and related disabilities
  • autism spectrum disorder
  • traumatic brain injury
  • spinal cord injury and similar disability
Assists with addressing the immediate case management needs of individuals in complex and emergent situations as determined by DDSN, including but not limited to those individuals who are judicially admitted to DDSN services, abandoned at hospitals, discharging from current placement with limited or no discharge options, and those who are in crisis in community placements.
  • Provides comprehensive case management services for assigned cases.
  • Assists with implementing needed services and supports for stabilization.
  • Authorizes services as approved on Support Plan.
  • Facilitates transfer to identified case manager provider once the case has been stabilized.
  • Documents activities as required and provides requested updates to supervisor.
  • Other case management functions as assigned.

Minimum and Additional Requirements
The candidate must possess a bachelor's degree from an accredited college or university and at least one (1) year of experience working with the target population.
Preferred Qualifications
A graduate degree from an accredited college or university and experience in the DDSN service delivery system. Experience delivering case management services is preferred.

Immunization: If this position requires the applicant to work directly with DDSN consumers, a (2) step PPD testing or QuantiFERON TB Gold testing must be completed prior to beginning employment. All new employees who are healthcare providers are required to provide documentation of immunity or be immunized against measles, mumps, rubella, pertussis, Varicella and Hepatitis B prior to beginning employment.
State Disaster Plan: In accordance with the State's Disaster Plan, which includes hazardous weather, DDSN employees may be required to work in times of an emergency or disaster.
