Full-time (37.5 hrs. per week) position, starts at $20.48/hr. - $21.50/hr. DOE with COMPETITIVE BENEFITS INCLUDING:

Compensation is set by agreed to language in the Collective Bargaining Agreement between CCSNW & SEIU 1199NW and determined based on experience.

 

Incumbent is responsible for providing direct service Case Support to Navigation clients and other needed services including but not limited to referrals to community services, help gathering documentation needed for housing and updating paperwork.  Incumbent is also responsible for providing overall support to the Navigation Team including but not limited to follow up with clients, filing of paperwork and making sure documentation is in order for referrals.


MAJOR DUTIES AND RESPONSIBILITIES:

 

CASE SUPPORT/ENGAGEMENT SERVICES:

  • Work with clients to remove barriers to access services, such as transportation and daycare.
  • Engage with new clients to see how progress is on their action plans. Offer support and problem solve around any issues they may have, to keep them engaged.
  • Follow up with individuals who are not connecting with their navigator and try to keep them active in the housing process.
  • Work to help clients obtain documentation for housing referrals.
  • Responsible for assisting clients in accessing community resources such as food banks, shelter, transportation, medical care, employment and training/job search, etc.
  • Responsible for monitoring time between contacts with navigation services and exiting clients when appropriate.
  • Communicate all engagement work done with clients to their primary navigator.

ADMINISTRATIVE SUPPORT:

  • Assist navigation team with client record management:
    • Maintain accurate and timely records on each client according to the Agency’s client record policies and procedures.
    • Input and maintain accurate and timely data within the HMIS record system.
    • Prepare and submit timely reports to internal billing department and other entities as requested.
    • Responsible for coordinating with staff and other agencies and organizations to expedite service connection for each client according to their action plan.
    • Attend staff meetings and training as required.
    • Undertake special projects and job assignments as requested by the supervisor.
    • Work collaboratively and respectfully with staff in all disciplines to develop continuously improving client services. 

HMIS – Incumbent is expected to develop proficiency in maintaining accurate client records in HMIS, the client tracking software application used by programs throughout the agency.   Proficiency includes entry of data into the client record, the editing of information as needed, and the printing of reports within HMIS.

MINIMUM:

  • 6 months experience working in homeless services, with individuals and/or families trying to access community resources or equivalent field.
  • Experience working with challenging populations in a fast paced environment.
  • Requires the ability to establish and maintain effective working relationships with staff, community partners and professionals.
  • Computer experience including data entry.
  • Excellent communication skills (both verbal and written).
  • Requires time management and interpersonal skills.
  • Reliable transportation, Current WA State Driver’s License and Proof of current WA Auto Insurance.

PREFERRED:

  • Case Management experience.
