- Excellent organizational skills to handle a substantial patient caseload.
- Experience or a willingness to learn scheduling for terminally ill patients.
- Possession of a strong value system, showing loyalty and dedication to serving humanity.
- Holding a valid driver’s license, car insurance, and dependable transportation for patient visits and doctor appointments.
- Assist in the preparation and processing of patient assessments and care plans.
- Collaborate with patients and their healthcare providers to handle medical documentation for their home care renewals every six months.
- Coordinate with providers to secure approval for the patient's appropriate durable medical equipment (DME).
- Attend patient doctor appointments, provide support, and advocate for suitable home care and DME.
- Conduct visits to patients' homes to build and strengthen relationships with the patient and their family members.
- Provide necessary training to patients, home health aides, and nurses as required.
- Conduct weekly check-ins with all team members, nurses, and home health aides involved in caring for each patient under your management.
- Collaborate and manage a caseload of 150-200 patients as a team.
- Some night/weekend calls may be required based on the severity of the patients' and the care teams caseload.
