  • Quick adaptability to learn various technological programs. 
  • Ability and willingness to adapt to constant changes within the organization. 
  • Excellent organizational skills to handle a substantial patient caseload. 
  • Experience or a willingness to learn scheduling for terminally ill patients. 
  • Possession of a strong value system, showing loyalty and dedication to serving humanity. 
  • Holding a valid driver’s license, car insurance, and dependable transportation for patient visits and doctor appointments. 

  • Responsibilities:
  • Assist in the preparation and processing of patient assessments and care plans. 
  • Collaborate with patients and their healthcare providers to handle medical documentation for their home care renewals every six months. 
  • Coordinate with providers to secure approval for the patient's appropriate durable medical equipment (DME). 
  • Attend patient doctor appointments, provide support, and advocate for suitable home care and DME. 
  • Conduct visits to patients' homes to build and strengthen relationships with the patient and their family members. 
  • Provide necessary training to patients, home health aides, and nurses as required. 
  • Conduct weekly check-ins with all team members, nurses, and home health aides involved in caring for each patient under your management. 
  • Collaborate and manage a caseload of 150-200 patients as a team. 
  • Some night/weekend calls may be required based on the severity of the patients' and the care teams caseload.
