RESPONSIBILITIES:
While working from home, the Case Manager/Service Coordinator identifies and monitors the services provided to participants through OLTL and applicable regulations to determine quality of services provided and verifies whether the type, amount, scope, duration and/or frequency of services need to be adjusted.
In partnership with the participant, supports an individual’s goals to live in the most integrated community setting as independently as possible and to establish natural community connections. Will support participants in Lycoming and surrounding counties to receive services in the time frame established by program guidelines and with the Agency’s established standards.
Provides necessary training and support to participants and consumer employers in the areas of managing direct care services that will enhance participant satisfaction of services.
Prepares and completes all forms and documentation as required in HCSIS, SC Database and the HALO system to ensure accurate and timely information.
Completion of Daily Activity Report and Allocations to assure appropriate program billing/allocation is met.
EDUCATION/EXPERIENCE:
Bachelor's Degree including or supplemented by at least 12 college-level credit hours in sociology, social welfare, psychology, gerontology or another behavioral science AND
One year of case management experience, OR
A combination of experience and training which adds up to 4 years of experience and education which includes at least 12 semester hours of college-level courses in sociology, social work, social welfare, psychology, gerontology or other social science.
