Job Summary:
We are seeking a Qualified Mental Health Specialist (QMHS) to join our team and provide comprehensive case management services to clients with Mental Health needs. The QMHS will be responsible for coordinating care, advocating for clients, and providing support services to ensure the best possible outcomes. The ideal candidate should have excellent communication skills, experience working with clients with Mental Health needs, and a strong commitment to improving the lives of others.
Responsibilities:
- Ensure that clients receive highly effective care delivered with compassion and integrity
- Follow and advance the company’s mission, vision, and values
- Conduct and/or assist in group sessions
- Follow individualized care plans for clients with Mental Health needs
- Coordinate, monitor clients’ Mental Health services, such as medication management and therapy sessions
- Advocate for clients and connect them with community resources, such as housing and employment
- Conduct regular assessments of clients' progress and adjust care plans as needed
- Provide crisis intervention services as needed
- Maintain accurate and up-to-date client records and documentation
- Collaborate with other healthcare professionals to ensure comprehensive care for clients
- Attend trainings and conferences to stay current on best practices for case management services
- Adhere to ethical and legal standards for case management services
- Maintain confidentiality and privacy of client information

Pay: $30.00 - $35.00 per hour
Expected hours: No less than 20 per week
