Summary:
Provides triage services for persons experiencing homelessness. Works closely with medical behavioral health care to help identify the type(s) of additional health/social services an individual is in need of and connects the patient to these supports. Heritage Health staff have an active role in our Patient Centered Medical Home model of care. This role is designated as part of the Heritage Health PCMH Care Team.
Requirements
Minimum qualifications:
Bachelor's degree in health or human services and experience in the field of mental health and addictions. Community outreach experience, preferred. Working knowledge of administrative procedure of a federally funded community health center, preferred. Computer skills necessary to maintain client charts and communication within the organization. Must be able to pass Idaho Department of Health and Welfare Criminal History Unit enhanced background check.

Essential Functions:
- Provides triage, crisis intervention, resource connection, navigation, and support to persons experiencing homelessness. These activities take place in the community and in an office setting.
- Coordinates with crisis centers, psychiatric and medical hospitals, agencies, treatment centers, and the criminal justice system.
- Addresses immediate needs and/or potential risk of crisis.
- Establishes and maintains relationships with all HH departments, partners, and relevant outside organizations, including as an active participant in the Homeless Coalition.
- Provides community education through outreach and presentations.
- Coordinates identified care needs with appropriate supportive services.
- Coordinates continuity of care.
- Communicates effectively with members of the treatment team.
- Demonstrates a commitment to patient centered care.
- Maintains accurate documentation in electronic charts, including completion of case management assessments and encounter notes.
- Provides all services in a professional and respectful manner at all times.
- Compliance with confidentiality policy as it applies to protection of client's rights, HIPPA, 42 CFR Part 2, and PHI.
- Makes CPS (Child Protection Services) referrals when necessary.
- Participates in performance improvement activities as assigned.
- Understands and adheres to all Heritage Health/Behavioral Health Policies and Procedures.
- Attends all staff, provider meetings and other meetings as necessary.
- Regular and predictable attendance is an essential function of this position.
- Carries out other related duties as assigned.
- Daily travel is required.
