A Magnet-designated healthcare system in Houston, TX is searching for an inpatient Case Manager to join its growing team. In this role, you will support interdisciplinary teams and facilitate patient care with the objective of enhancing the quality of clinical outcomes and patient satisfaction while managing the cost of care and providing timely and accurate information to payors. Responsibilities:
- Coordinates/facilitates patient care progression throughout the continuum.
- Works collaboratively and maintains active communication with physicians, nurses, and other multi-disciplinary care team members to ensure quality patient care.
- Completes treatment and discharge plans, modifies plans of care as necessary to meet the ongoing needs of the patients, and communicates to third-party payors and the care team as needed.
- Completes Utilization Management and Quality Screening for assigned patients.
- Addresses/resolves system problems impeding diagnostic or treatment progress.
Qualifications:
- A BSN or MSW is required.
- Active RN License or LMSW/LCSW
- 3+ years of acute care experience is required
