Title: Case Manager

Women’s Empowerment is a community-based organization whose mission is to educate and empower women, who are homeless, with the skills and the confidence necessary to secure a job, create a healthy lifestyle and regain a home for themselves and their children. Women’s Empowerment is the most comprehensive job-readiness program in Sacramento exclusively designed to serve homeless women and their children. We are dedicated to ending homelessness through empowerment and employment.

Women’s Empowerment is launching a transitional housing program for families with 24 stand- alone cottages in a gated community. Women’s Empowerment graduates work hard to secure jobs and need housing they can afford with mid to entry-level salaries. This buoys their success and allows them to continue on that trajectory, building their credit, beginning a savings account, putting down roots in their job, and eventually securing a promotion that allows them to qualify for market-rate housing. This project provides the critical stepping stone they need to reclaim their lives and futures, empowering them to define themselves by who they are and what they contribute to their community rather than by their lack of housing.
Job Summary:
The Case Manager is responsible for providing case management and support services to women who have previously experienced homelessness. The Case Manager will work closely with clients to assess their needs, develop individualized service plans, and connect them with community resources to help them maintain housing stability.
Key Responsibilities:
- Conduct initial assessments to identify members of the household’s needs and strengths
- Develop individualized service plans that address members of the household’s needs and goals
- Provide ongoing case management services to ensure all members of the household are connected to necessary resources and services
- Advocate for all members of the households needs with community organizations and government agencies
- Monitor members of the households progress towards their goals and adjust service plans as needed
- Maintain accurate and up-to-date case records
- Provide crisis intervention and support as needed
- Collaborate with other service providers to coordinate services for all members of the household
- Participate in regular staff meetings and trainings
Qualifications:
- Bachelor's degree in social work or related field preferred
- Experience working with homeless or formerly homeless populations
- Knowledge of local community resources and services
- Strong communication, problem-solving, and organizational skills
- Ability to work independently and as part of a team
- Flexibility and adaptability to changing situations
- Valid driver's license and access to reliable transportation
Physical Demands:
The position requires the ability to sit for extended periods, use a computer and telephone, and perform light lifting (up to 25 pounds).

Job Type: Part-time
Pay: $20.85 - $25.11 per hour
Expected hours: 24 – 30 per week
