Reports to: Case Management Supervisor
This person provides field-based targeted case management and adjunctive services to a broad variety of older adult clients, including:
1. Provide clinical outreach services to older adult community residents with mental illnesses.
2. Network with referring professionals.
3. Contact older adults needing interventions.
4. Develop ongoing relationships of trust.
5. Implement a wide variety of interventions to improve clients' mental health, including crisis management, providing referrals, problem solving, mental health case management, and accompanying clients to medical appointments or service agencies to secure needed assistance.
6. Help clients identify and access resources, including housing.
7. Assist in the establishment of benefits for which the clients are eligible.
8. Perform other tasks as assigned.
Services will be provided in the clients' homes, in the community and in the office. All services provided will be consumer-centered, culturally competent, and will utilize Bio-psychological Rehabilitation principles. Case Manager must demonstrate patience with older adults, be aware of culturally appropriate behavior and its effects on the responses of his/her clients, and flexible according to the needs of the client. Clinical supervision will be provided as appropriate to legal requirements and Case Manager's need.
Other duties and responsibilities:
1. Meet 100% of productivity requirement.
2. Maintain appropriate DHCS documentation.
3. Other agency tasks as assigned.
PHI Access Level:
ICMS Case Managers have access to their own ICMS client charts on a regular basis and can discuss ICMS shared clients with clinicians, DMH Case Managers, other clinical staff, and ECM Case Managers when there is a Universal Consent Form.
Qualifications:
Must have a bachelor's degree in a mental health field (e.g., psychology, social work, marriage and family). Experience working with older adults is preferred. Must have appropriate computer and word processing skills and be able to handle multiple simultaneous tasks. Must demonstrate good interpersonal and communication skills, be able to maintain confidentiality, and present a professional appearance.
The following are essential to employment in this position:
- Own, and have readily available for use, a licensed, working vehicle;
- Valid California driver's license;
- Current vehicle insurance that meets minimum liability limits prescribed by California law, and eligible for coverage under Heritage Clinic’s secondary auto insurance;
- Evidence of a negative tuberculosis examination within 60 days prior to employment; and
- Illegal drug-free status indicated by pre-employment drug screening and post-employment individualized suspicion testing and post-accident testing.
