Children's Home Shelter for Family Safety , in Sioux Falls, SD is currently seeking a Full-Time Case Manager to provide case management services to children, youth, and families within the Community Based Services programs in accordance with the organization's Mission, Vision, and Values. Responsibilities include assisting in the overall operation of the program.
What You'll Do:
Your responsibilities as a Full-Time Case Manager would include:
- Complete assessments and other documentation during the child's evaluation phase of treatment.
- Develop individualized case plans, prepare case plan reviews and discharge summaries with treatment team. Consult with psychiatrist and psychologist.
- Provide case management services to children, to include regular and consistent home visits, arranging family contacts/visitation with placing agency, and overseeing the child's treatment and care in home and school.
- Provide case management services to families, including family support services to individual family members, as well as parent training.
- Provide transitional and aftercare services.
- Maintain communication and work in cooperation with referral agencies.
- Provide evaluation services for children and families as directed by Agency policy.
- Perform court work as necessary to advocate on behalf of a child to help facilitate permanency.
- Provide supervision of children as necessary.
- Assist in recruitment of foster and adoptive parents/families.
- Assist in the development and facilitation of monthly training for foster families.
- Collaborate with other program personnel to ensure treatment and licensing staff have pertinent information necessary for each family in the program.
- Provide on-call duties as needed.
- Attend required annual training. Maintain required certifications and proficiencies.

Schedule:
- Base Monday through Friday 8 to 4 pm work week with Hybrid capability.
Location:
- Rapid City base location with Hybrid capability
Compensation:
- $43,000 to $55,500 (depending on Education and Experience)

Requirements
Experience/Education/Skills:
- Bachelor's Degree required.
- Master's degree in social work, or related field, desirable.
- Minimum of South Dakota Social Worker Associate License required.
- Two years' experience in children and/or family programs.
Physical Demands and Work Environment:
Environment focused on care of individuals in crisis situations. The employee is regularly required to bend and sit on the floor. Must regularly lift and/or move up to 10 pounds and occasionally lift and/or move up to 50 pounds. While performing the duties of this job, the employee must be able to physically restrain a child in ways taught by the Agency while being able to verbally communicate to the child and other staff.
