Case Manager

GENERAL DESCRIPTION:

The Case Manager is responsible for developing, implementing, and evaluating medical care plans in conjunction with the patients’ self identified needs. An essential goal is to help a patient to develop a self management individualized care plan. Within the patient medical home model, the Case Manager will serve as a liaison between the Agency, Health Care Providers, patient and their families.

In addition, this position requires:

Education:

• Graduate of an accredited RN program. Preferred Bachelor of Science degree

Work Experience:

• Minimum of three (3) years nursing experience
• Preferred work experience as a Case Manager or in a community-based program

Licensure and Credentials:

• Current SC nursing license
