Job Description
GENERAL DESCRIPTION:
The Case Manager is responsible for developing, implementing, and evaluating medical care plans in conjunction with the patients’ self identified needs. An essential goal is to help a patient to develop a self management individualized care plan. Within the patient medical home model, the Case Manager will serve as a liaison between the Agency, Health Care Providers, patient and their families.

In addition, this position requires:
Education:
- Graduate of an accredited RN program. Preferred Bachelor of Science degree
Work Experience:
- Minimum of three (3) years nursing experience
- Preferred work experience as a Case Manager or in a community-based program
Licensure and Credentials:
- Current SC nursing license
Skills:
- Excellent verbal and written communication skills
- Demonstrated leadership skills
- Current computer skills to include Microsoft Office
- Ability to establish and promote positive, interpersonal relationships
- Excellent organizational skills
- Cultural awareness and appreciation for differences
- Strong medical/nursing knowledge-base

Physical Abilities:
- Able to push, pull and reach, sit, stoop, stretch, and stand

- Required to talk and hear
- Have the hand-eye coordination and manual dexterity needed to operate a computer, telephone, copier, and medical equipment
