POSITION TITLE:Case Manager
REPORTS TO: Regional Case Manager
POSITION STATUS Full-Time, Exempt
JOB DUTIES AND RESPONSIBILITIES
· Communicate regularly with clients and their families regarding treatment.
· Advocate for options within the client’s best interest.
· Promote patient safety and quality of care.
· Collect statistical data on the effectiveness of various therapies and programs.
· Maintain all required documentation, records, and reports.
· Partner with social and human service organizations to administer client care.
· Educate clients about available resources, community programs, and program-specific outcomes.
· Identify social, economic, and physical needs and barriers with clients; assesses client's support systems, available community resources and other factors to develop an appropriate case plan. (For reassessments.)
· Provide casework services to individuals in the office and/or in the client’ s home, in the areas of crisis intervention, stabilization, and goal planning.
· Maintain all required documentation, records, and reports.
· Prepare statistical reports and summaries.
· Read and respond to outlook emails within 24 hours.
· Provide supervision and support to troubleshoot and resolve client/staff issues.
· Approve supervisee's progress notes and timesheets.
· Facilitate community referral and follow-up with, on, or behalf of the client.
· Explain agency and program guidelines, regulations, and procedures; assists clients in completing required forms and in gathering necessary documentation.
· Conduct reassessments and updates recovery plan weekly.
· Create and maintain caseload spreadsheets.
· Mentor/train new Case Managers.
· Participate in regular staff meetings, staff training programs, supervisory sessions, and accept the responsibility of aiding the development of positive team relationships.
· Other duties as assigned.
Education/ Experience:
· Master’s degree in social service (preferred)
· Bachelor’s in social service (required)
· CANS/ANSA (preferred)

INITIAL APPROVAL & COMPETENCYThe above employee has demonstrated the capability to satisfactorily perform the responsibilities of this position. All Positions are on a 90- day probationary period at which point employee will be re-evaluated to document training has been effective and competency approved

Pay: $43,000.00 - $50,000.00 per year
