Case Manager

Summary:

One of our exclusive partners is expanding their program and seeking (8) Care Managers to join the team ASAP! These are permanent, full-time jobs and are based out of Bronx, NY.

Salary depends on experience and degree:

• Masters Level: $50,000 - $55,000
• Bachelors Level: $45,000 - $50,000
• Associates Level $38,000 – $46,000

Responsibilities:

• Oversees and provides access to all of the services an individual needs to assure that they receive everything necessary to prevent hospitalizations, stay healthy, and maintain stability.
• The Care Manager is ultimately responsible for the overall provision and coordination of services to assigned caseload.
• The Care Manager guides program enrollees and their caretakers (legal guardians) through the health care system by assisting with access issues, developing relationships with service providers, and tracking interventions and outcomes

Requirements:

• Must have a Degree in Human Services (Psychology, Sociology, Social Work, etc.)
• 1+ year of Case Management/Care Management exp preferred
• Experience with children and adolescents
• Bilingual (English/Spanish) and plus!

Job Type: Full-time

Pay: $40,000.00 - $65,000.00 per year
