Position Details
The Dual Diagnosis Treatment Team (DDTT) program offers support to children and adults who have both a serious mental illness and a developmental disability. Participants receive counseling, physical healthcare, support finding housing, assistance during crises, and help dealing with other issues in their daily lives.
Case Managerprovides information and care coordination during the Initial Treatment Team Meeting, and 30-day ITT meetings thereafter; care coordination activities are deliberated with individuals served and their supports.

Requirements:The ideal candidate will possess the following qualifications:
- Completed Bachelor's degree in social work, psychology, or another human service related field required.
- One year of experience in a mental health setting and one year working in a developmental disability setting preferred.
- Driver's license required.
