The Sheriff has announced an opening for a Case Manager in the Lafourche Parish Correctional Complex Regional Reentry Program at the Day Reporting Center. The Case Manager provides guidance and structure to formerly incarcerated participants enrolled in the Day Reporting Program.
Applicants must have:
· Bachelor’s degree in Human Services discipline or equivalent;
· Familiar with community corrections systems and practices;
· Knowledge of community workforce needs;
· Knowledge of community resources providing basic and extensive human services;
· Ability to work a flexible schedule;
· Computer literacy with fluency in the case management and assessment programs;
· Good writing and presentation skills;
· Empathy and compassion in working with corrections transition population;
· Ability and willingness to gain expertise in the area of reentry and rehabilitation.
The essential duties and responsibilities include, but not limited to the following:
· Coordinate personalized case plans with measurable goals for all assigned participants based on their risk/need assessment. Case plans will be reviewed with the participant on their changed progress within the program;
· Welcome participants to the program, assist with the Intake process and paperwork, as well as, orient participants to the facility, program rules and the classes that will be covered;
· Facilitate cognitive based lessons in a structured classroom while following a curriculum and promoting engagement from the participating individuals;
· Document the status on each case, progression within the program and involvement in the classes;
· Maintain confidentiality of all sensitive information while working with other staff, community service providers and participants enrolled in the program;
· Capable of following a timely program schedule, reinforcing the program schedule to participants and hold the participants accountable for class attendance;
· Comply with all policies, customer contract agreements in order to provide effective treatment and services;
· Assist participants with identifying barriers and referring them to outside community resources that can guide them to a successful pathway;
· Assist participants towards a successful employment plan with a focus on soft skill training and job readiness needs;
- Complete and update enrollment paperwork for prospective students;
- Maintain separate student folders with necessary documents;
- Obtain required test security training for standardized proctoring;
- Provide orientation to newly enrolled students;
- Provide tutoring or education resources to students for in-person or distance learning hours;
- Schedule evening and virtual classes to accommodate working students;
- Create lesson plans and individualized student learning plans;
- Monitor, track, and update education hours, scores, and benchmarks according to the SIS spreadsheet.
Applicants will appear before an Oral Review Board, which will be scheduled in the week following the deadline date. The Oral Review Board will consider, but will not be limited to the following information: years of service, college education, and military service, training certifications, special skills and dependability.
Beginning annual salary will be $40,000.00. This position is not eligible for State Supplemental Pay or a take home unit.
