Job Description
The Case Manager is responsible for the development, coordination, implementation, and monitoring of services provided to persons with intellectual disabilities in a HCS (Home and Community Based Services) setting, in accordance with HCS principles. Case Manager will serve as the primary program contact for the HCS Program. The Case Manager will participate in the annual PDP/IPC meetings, monitor service delivery, identify problems, and coordinate efforts to assure best implementation.
Client Services
Responsible for new intake for all clients.
Process all Monthly and Quarterly reports for clients.
Monitor all clients' medication MAR.
Maintain, renew and update client binders, documentations, and Incident Reports.
Report and document all allegations of Abuse, Neglect and Exploitation.
Process client’s monthly eligibility through THMP system.
Transport clients to all appointments when necessary or required.
Supervise and monitor the safety and security of all clients. Follow incident processes and reporting requirements.
Track, Monitor, and record all client movement in the group homes on the TMHP and Focused Portals.
Responsible for all In-services for new clients and staff.
Train, Supervise, and approve all in-services and SDLs.
Quality and Compliance
Guarantee that documentation of all supports delivered to participants is accurate and timely in accordance with VOATX policies and the applicable HCS/TxHmL billing guidelines as well as state and federal regulations.
Deliver all aspects of operational standards, to include, but not limited to IPC & PDP renewals and revisions. Set up new client binders, services, and medical profiles.
Monitor MDCS’ compliance and performance outcomes for partner agencies.
Ensure all service sites are in compliance with state and federal regulations without punitive actions.
Consistently seek ways to improve services utilizing feedback from individuals served, surveys, data and stakeholders. Attend team meetings, visit sites, and spend time with various stakeholders
Responsible for preparedness for all audits

Requirements
- Proven knowledge of case management and Medicaid waiver programs.
- Bachelor's or related field plus one year experience IDD individuals OR a high school diploma/GED plus five years direct experience with IDD individuals OR the equivalent combination of education and experience.
- Documentation skills. Great verbal & written communication skills
- Excellent organizational and time management skills
