KVC Kansas is hiring a Case Manager! This role involves creating tailored program and treatment plans to address the health and wellness requirements of our clients. Responsibilities include setting individual treatment goals, organizing care, collaborating with birth/foster families and other community providers, and overseeing staff and youth in residence while offering training. ESSENTIAL DUTIES:
- Manage client intakes and department collaboration.
- Complete compliance reports.
- Ensure licensing compliance.
- Review variance and incident reports.
- Help monitor staff supervision.
- Supervise children's treatment.
- Develop and apply program schedules and behavior systems.
- Keep in touch with staff and clients daily to ensure they follow the schedule.
- Ensure structure through behavior systems and house expectations.
- Provide feedback on staff performance.
- Coordinate youth program with agencies and resources.
- Keep resources for identified needs.
- Document community resource usage.
- Complete progress notes weekly and monthly.
- Provide aftercare for discharged clients.
- Plan treatment and discharge coordination.
- Participate in administrative on-call rotation.
PREFERRED EXPERIENCE/SKILLS:
- BSRB licensure.
- Experience in group home/residential settings or as a Case Manager.
- Supervisory experience in childcare or juvenile justice agencies working with similar youth.
- Proficiency in office technology, such as electronic health records and Microsoft operating systems.
EDUCATION:
- Bachelor’s degree in a human service field (social work, psychology, human development and family life, criminal justice, counseling), nursing, or education.
- Or relevant experience will be considered with approval from the facility’s licensing body and/or state regulators.
LICENSURE/CERTIFICATION:
- Valid driver’s license
- Auto insurance
