This position is eligible for personal medical coverage beginning the first of the month after the hire date and has an assignment lasting no more than one year.
Description:
The Case Management Associate performs substantial support work providing services related to case initiation and caseload management within the Single-Entry Point unit. The position is responsible for completing the initial assessment of case status, determining case routing, answering intake phone line, processing referrals, and returning calls, and ensuring effective communication and cooperation between Single Entry Point, Long Term Care Eligibility, and Community Service Providers.
This position typically operates on a 5-day work week, Monday-Friday in a hybrid work model. The work schedule will be discussed more during interviews.
Consistent attendance and punctuality are a priority due to the coverage needs of this role.
Target Hiring Range: $25.09 - $26.00 USD Hourly
Compensation will be determined based on education, experience and skills.
Benefits: The employment status is Limited (Fixed-Term) with an anticipated assignment end date after one year and is eligible for personal medical coverage following the month after the employee's hire.
Essential Duties:
  • Receives and processes new applications and referrals and related supporting documents within state mandated time frames, manages referrals of pending HCBS cases, and gathers and documents information on financial eligibility status from CBMS.
  • Researches internal workflow databases for history and status of identified parties in applications and records information into the state data system. Conducts complete case reviews, determines appropriate case routing, and transfers cases to Case Managers.
  • Conducts research utilizing various computer-based applications including BUS, Interchange, CBMS, and GenApp.
  • Assists with reviewing changes or additions to procedures and policies to confirm the workflow and identify potential issues.
  • Documents and processes referrals and billable units in the Business Utilization System within state mandated timeframes to support records maintenance and reporting.
  • Assists customers in completing the application process and in navigating other Human Services programs and community resources to ensure families are receiving needed assistance and services. Refers customers to appropriate resources.
  • Communicates internally and externally to ensures effective collaboration and cooperation between Single Entry Point, Long Term Care Financial Eligibility, Community Service Providers and other referring agencies.
  • Provides orientation coordination for new employees of the Single Entry Point unit.
  • Orders and maintains unit supplies and equipment.
  • Other duties and responsibilities as assigned.

Minimal Qualifications:
  • High School Diploma, GED or equivalent certificate AND
  • 1 year of work-related experience
  • OR an equivalent combination of education and experience

Preferred Knowledge, Skills and Abilities:
  • Strong written and oral communication
  • Ability to articulate clearly and effectively to varying levels of customer
  • Experience with Microsoft Office/Microsoft 365
  • Strong time management and prioritization skills
  • Ability to accurately compile, format, and file electronic data
  • Proficiency in processing paper and electronic records per established records management protocols
  • Provide other administrative and technical support as needed
  • Reliable travel for coverage of on-site needs is strongly preferred
