Mission Statement:
Customized Assistance Services (CAS) helps Human Resources Administration (BRA) clients with health and/or mental health problems reach the highest attainable level of functioning and self-sufficiency by providing comprehensive, integrated, individualized clinical and support services. CAS works with other components of HRA and with other governmental and non-governmental service providers to create new programs and to integrate and refine existing services so the people it serves can achieve their maximum functional capacity. CAS provides clinical expertise, recommendations, and direction to HRA in the fields of health, mental health, substance abuse and vocational rehabilitation. CAS staff provides psychiatric evaluation and crisis intervention services, housing eligibility services and assistance in obtaining federal disability benefits. CAS manages contracts that serve individuals and families with medical, mental health, and/or substance abuse disorders. CAS programs provide clinically focused case management and utilize sophisticated clinical tracking and reporting systems. The Case Management Services Director will administer, coordinate, and monitor SUPPM Case Management (CM) Programs which provide for the delivery of comprehensive CM services for all populations designated by HRA.
The Disability Services Program assists clients in obtaining federal disability benefits through the Social Security Disability Insurance program (SSI/SSDI). CAS is looking to hire eight (8) Caseworkers who will:
-Interview and screens clients to assess their potential eligibility for federal disability benefits by completing required Social Security Administration (SSA) documents. Assists potentially eligible clients to apply for federal disability benefits and/or appeal SSA denied applications. Obtains additional relevant medical and mental health documentation to support client's SSA application and potential appeal.
-Review SSA initial applications, denial reasons and accompanying documentation. Collects and prepares current medical evidence to validate appeal status on behalf of claimant.
-Evaluate clients' need for ancillary services, prepares required referral documentation and makes appropriate referrals to other HRA and non-HRA services/ programs. Collaborates with WeCARE vendors, HASA and other programs to identify and enhance a clients' eligibility for federal disability benefits. No fieldwork required.
-Utilize and update agency's electronic record system to schedule clients' appointments for SSA initial applications and appeals, to monitor and follow-up on clients' compliance, to document outcomes and input assessment data.
-Review mandated notices, reschedules mandated appointments within required timeframes, and documents outcomes electronically. Schedules appointments for SSA initial and appealed applications.
-Counsel clients regarding the benefits of appointing an authorized representative during the appeal process.
Qualifications
1. A baccalaureate degree from an accredited college or university accredited by regional, national, professional, or specialized agencies recognized as accrediting bodies by the U.S. Secretary of Education and by the Council for Higher Education Accreditation (CHEA).
Additional Information: Section 424-a of the New York State Social Services Law requires an authorized agency to inquire whether a candidate for employment with child-care responsibilities is or has been the subject of an indicated child abuse and maltreatment report on file with the State Central Register for child abuse and maltreatment. State Central Register screening will be conducted prior to considering a candidate for employment as a Caseworker. Candidates who have been the subject of an indicated child abuse and maltreatment report will not be assigned to any position which requires child-care responsibilities. Employees who have been the subject of such a report may be reassigned or terminated from employment from their positions as Caseworker.
