POSITION SUMMARY

The Transitional Living for Young Adults Program (TLYA) and TLYA Case Coordinator plays a key role in Guidance Center of Lea County’s (GCLC) mission to support youth and families to thrive in their communities. This position works closely with other GCLC Case Coordinators, clinical services, and community partners to provide collaborative, comprehensive services.

This role provides program oversight and case management for young adults ages 16½ - 23 in a congregate care shelter and potential scatter sites within Lea County. Case management includes accessing resources, connecting with community, increasing well-being, engaging with vocational opportunities, and growing resilience through case management.

RESPONSIBILITIES

Direct Services (70%)

  1. In partnership with participants, develop life maps (case plans) with a goal of promoting independence and a solid foundation.
  2. Meet individually with participants to:
    • Assess basic needs for shelter, food, clothing, physical and mental health
    • Identify appropriate resources and advocate for services
    • Assist participants to secure identified resources
    • Support participants in accessing longer-term care supports
    • Provide crisis intervention services as needed
    • Assist youth in developing life skills and positive coping skills
  3. Conduct Ansell Casey Life Skills assessment and develop individualized plans for each resident which are updated quarterly.
  4. Provide ongoing feedback of individual’s movement toward achievement of their transitional goals, both to the individual and the service team.
  5. Utilize solution focused strategies and motivational interviewing techniques to assist individuals in progressing toward their identified goals.
  6. Once housed through TLYA, continue to assist clients in applying for and obtaining mainstream benefits including NMHHS and Section 8.
  7. Maintain consistent home visits with youth.
  8. Once households are housed, provide a minimum of monthly case management.
  9. Research, network, and provide connections for residents to become engaged community members through social and volunteer opportunities.
  10. Input daily progress notes and transitional living plan updates.
  11. Assist with the maintenance of client records.
  12. Assist with the collection of client activity schedules as needed.
  13. Assist with occasional transportation needs of clients for medical, State, personal and other case management demands.
  14. Commit to the values of positive youth development, harm reduction, transformative justice, anti-oppression and trauma informed care.
  15. Travel to client apartments to deliver services to youth in community.
  16. Meet weekly with Program Manager for one-on-one supervisory check-ins.
  17. Perform other duties as assigned by the Program Manager/Director of Clinical Services.

General Responsibilities:

  1. Preserves the integrity of the programs and agency.
  2. Attend scheduled Program and All Staff meetings.
  3. Demonstrates commitment to GCLC’s and TLYA Program’s mission and vision.
  4. Supports overall growth of the agency.

LEVEL AND DEGREE OF SUPERVISION:

Supervised by Humphrey House Program Manager with clinical supervision provided by the Director of Clinical Services. Supervision to include all in-service training sessions, client staffing, team meetings, general staff meetings, and specialized training as required by program.

QUALIFICATIONS:

Education and Experience:

  • Bachelor degree in Social Work or Youth Development preferred, or related field.
  • 2 years of experience working with homeless or at-risk youth.
  • Experience working with youth who have faced significant barriers and/or homelessness.
  • Experience completing assessments.
  • Experience creating goal/treatment plans.

Skills, Knowledge and Abilities:

  • High level of self-awareness and ability to work effectively with and relate to diverse populations.
  • Excellent verbal and written communication skills, especially for client advocacy.
  • Ability to work independently and show high aptitude for decision-making.
  • Demonstrated ability to efficiently organize, coordinate, track and complete multiple tasks as well as adjust to changing priorities.
  • Knowledge of child welfare systems, public and federal assistance programs, and other community resources.
  • Strong communication skills and experience having difficult conversations.
  • High level of attention to detail.
  • Desire to continuously learn for personal and professional growth.
  • High level of personal and professional integrity, including ability to protect and maintain confidentiality.
  • Computer application proficiency, including Google Docs and G-Mail.
  • Ability to be flexible in scheduling.
  • Must be able to pass Criminal Records Check and receive CYFD Clearance.
  • Access to reliable vehicle and ability to maintain good driving record and insurance levels as directed by GCLC, to fulfill job duties that require travel into the community.
