Job description

Job Description - Residential Program

Position Title: LPN

Location: Cortland, NY

FLSA Status: Non-exempt

Supervisor: Nurse Supervisor

Positions Supervised: None

Hours: Full-Time, 37.5/hours per week

Wage: Starting at $24.50/hour

Shifts: 6am - 5pm . Variable 6-2 pm, or a 7-3 pm or 8-4 pm or 9-5 pm. Always have Sundays off. Rotating Saturday coverage, hours on Saturdays are only 7am-1pm

Minimum Qualifications:

Must possess a valid New York State LPN or GPN, Able to pass CPR/First Aid training in first month of employment or possess current certification. Must be able to learn computer charting systems.

Desired Qualifications:

Two years' experience in a hospital/chemical dependency rehabilitation Treatment Program. The ideal candidate will possess a working knowledge of addictions, will be skilled in the use of medical equipment, vital sign capture/recording, labs, and medication management.

Key Tasks:

1. Reports pertinent observations in regard to individual patient care needs and makes suggestions regarding medical treatment plan and competently implement/instruct all care in the appropriate nursing practice to all staff with oversight by the RN.


2. Supervise the self-administration of medication and monitor the effects of medications including controlled substances and conduct accurate med counts. Order medications from the pharmacy in a timely manner as directed by physician order, to replenish stock, and/or refill patient medications.


3. Enter and discontinue medications, vitals, and implement other physician orders in the computer documentation system.


4. Accurately transcribe orders and treatments into paper MAR system and document medication dispensing.


5. Prepare patients for examination: height/weight, vital signs, collect and process lab specimens independently and/or in concert with contracted lab services.

Other Responsibilities:

· Serve as a resource to treatment team members.

· Offer patient education as determined by patient assessments/needs and treatment plans, if applicable.

· Review PRN reports and notify supervising RN, NP or MD of any abnormal findings.

· Monitor and/or perform diagnostic screening tests as ordered by the physician.

· Accurately and thoroughly document in the medical record utilizing approved medical record language and abbreviations adhering to the medical record policies and procedures.

· Provide thorough and accurate patient information during shift exchange for end of shift report.

· Schedule all patient appointments related to their medical treatment.

· Work a flexible schedule based upon service demands and work mandatory overtime as needed.

· Attend all mandatory trainings required for OASAS regulatory compliance and per CARS Personnel policies.

· Notify the RN/NP/physician in an appropriate and timely manner of changes in the patient's medical condition.

· Complete yearly training requirements related to licensure and assist in the onboarding of new medical staff.

· Other duties as noted and required to meet the emergent medical needs of the population served.
