Essential Duties and Responsibilities:
- Must have current CNA license for a minimum of 2 years
- The Field Supervisor will provide non-clinical guidance and training to home care aide staff in the field and will evaluate their performance in the field
- Reviews the Plan of Care and recommends an RN re-assessment based on the needs of client
- Evaluates the effectiveness of home care services by obtaining feedback from the client and/or the primary caregiver
- Submits assessments and other required documentation outlining the services rendered as indicated in a timely manner
- Participates in case meetings and interdisciplinary team meetings as scheduled
- Discusses any problems concerning the clients and/or the services being provided to the clients then developing appropriate and effective solutions
- Participates in office meetings and staff development trainings
- Assists with performance reviews of the aides and ensures competency
- Adheres to Federal, state and accreditation requirements
- Maintains licensures and/or relevant certifications as required
Qualifications:
- A current CNA certificate for a minimum of 2 years
- Must have reliable transportation
- Must have current driver's license
- Current vehicle insurance coverage
- Excellent interpersonal and coaching skills
- Ability to exercise professional discernment and maintain professional boundaries
